During our weekly job site visit to the Village Parkway Pump Station on May 20, 1997, the City indicated the
following items must t~ completed or addressed before a final walk through will be scheduled. This memo is
published for the benefit of the contractor and is not to be a final "punch list" nor comprehensive in nature.
The City reserves the right to add items to the list as required, or modify its content at completion of the
project.
l. On both days of scheduled instruction for the disinfection facilities, the City does not believe it received
adequate training. The City believes that instructional time was used by NTMCC to adjust the system and
that all adjustments should have been completed during the testing phase of the project. The City
contends that no instructions were given on the safety features of the facility as required by the contxact
specifications m~d that a general overview of the systems capabilities was not provided. Please schedule a
time through Mr. Matt Cast for completion of the training.
2. The driveway turn-out near Ground Storage Tank No. 2 continues to hold water. Positive drainage needs
be achieved prior to acceptance.
3. Modifications to the rail and ladder system used on the motor control platform have yet to be completed.
Since the steel steps and aluminum wall rail were removed from the project, aluminum handles are to be
placed on the two remaining ladders. In the location on the platform where the middle ladder was
relocated, the aluminum platform handrail shall be closed into one continuous piece.
4. All disturbed vegetation due to NTMCC construction shall be reseeded.
5. All debris on the exterior of the pump station shall be removed (construction materials, large concrete
debris, etc.).
6. We are enclosing a punch list of repairs required on the pump station irrigation system developed by the
City's Leisure Services Department.

We have received correspondence from the Engineer on the above. referenced project that if the City of Coppell
and the Engineer cannot have confidence that the work on the Motor Control Center and the $CADA System has
been completed and is correct by Friday, May 23, 1997 then we, North Texas Municipal Construction Company,
hc. will be held in default and liquidated damages will be assessed. This is not acceptable to North Texas
Municipal Construction Company, hc. (NTMCCD.
Accept this as your formal notification from NTMCCI that you are to do whatever it takes, work around the clock
if necessary, to get all the bugs on the MCC and SCADA corrected immediately. You will need to work closely
with Steve Baker and Joe Gibson with GE Testa Controls, be in constant touch with them and with North Texas
Municipal Construction Company, Inc. Team Control's offwe number is 972/241-6057; Steve Bakers pager is
