City of Coppell Post Office Box 478 Coppell, Texas 75019 Re: Village Parkway Pump Station Additions Proposed Change Order No. 4 & 5 Dear Mr. Griffin: We are enclosing two copies of proposed Change Orders No. 4 & 5 to the City' s contract with North Texas Municipal Construction Co. for the construction of the Village Parkway Pump Station Additions. Change Order No. 4 adds several items that have been identified during construction as needed on the project, yet not included in the original scope of the project. Change Order No. 5 adds components to the SCADA System as requested by the City. Both change orders more specifically describe the extra work proposed. Change Order No. 4 adds $2,991.00 to the contract amount which represents less than a 1/2% increase in the original contact amount. Change Order No. 5 totals $8,960.00, or an approximate 1% increase in the original contract amount. To date the total of all change orders on the project, including proposed Change Order No. 4 & 5, is $162, 158.00 or approximately 17.38% of the original contract amount. As a reminder, Change Order No. 2 added the SCADA System to this project and totaled $152,739.00 Please review the proposed change orders and notify us of your findings. The general contractor has stated he is ready to complete the project within the next few weeks if we can get these outstanding issues resolved. INTENT OF CHANGE ORDER The intent of this change order is to modify the provisions of the contract entered into by the City of Coppell, Texas and North Texas Municipal Construction, Inc., 5614 Dyer Street, Dallas, Texas 75206, for construction of the Village Parkway Pump Station Addition, dated January 9, 1996. B. DESCRIPTION OF CHANGE The City of Coppell desires to add the following additional items to the Village Parkway Pump Station Additions Project. These items were not included in the original scope of contract work: a) Furnishing and Installing lighting at the control panel for the disinfection system. The attached memo dated January 17, 1997 from MrCreary & Associates specifies the work and equipment required. b) Furnishing and installing Support System for the Cone Valves as shown on the attached Exhibit dated December 4, 1996 c) For Eliminating the staircase to the motor control center platform and modify the two access ladders to be no further than 8-inches from the edge of the platform. This work also includes the addition of a handrail across the opening at the original ladder location. d) For furnishing and installing insulation for the exposed chemical feed solution, sampling and waterlines at the disinfection facility. e) For furinshing and installing power feed to the overhead door opener. f) For furnshig and installing 4" diameter, liquid filled pressure guages on the discharge headers of Pumps No. 5 & 6. B. DESCRIPTION OF CHANGE The City of Coppell has requested the following additions or modifications to the SCADA System. The attached proposal from Team Controls more specifically describes the proposed equipment. a) Adding two (2) pressure point nodes at Coppell Road and Moore Road, utilizing existing phone data lines and existing equipment where possible. b) Adding an auto-switch over from the new level controls to the existing level controls at the Sandy Lake Road and DeForest Lift Stations. c) Adding HOA status input of each pump to the SCADA system at the Sandy Lake Road and DeForest Road Lift Stations. The modification requires the addition of an extra mixed I/O module at each site. This modification will also allow the City to monitor the rain gauge data at the Sandy Lake Road Lift Station. d) Installing the proposed RTU and antenna scheduled for the new elevated storage tank at the Sandy Lake Road Pump Station in order to monitor the Dallas Water Utilities flow rate into Coppell.
