We are enclosing two fully executed originals of Change Order No. 5 to your contract with the City of
Coppell, dated January 9, 1996, for the construction of the Village Parkway Pump Station Additions. Change
Order No. 5 covers the extra work required to install the pressure points at Moore Road and Coppell Road;
install the level transmitter auto switch for the Sandy Lake Road and Deforest Road Lift Stations; and to re-
establish the Dallas Water Utilities flow signal from the Sandy Lake Road Pump Meter Station.
These originals are for your files. You are hereby directed to proceed with the work covered by this change
order.

Construction Co., dated January 9, 1996, for the construction of the Village Parkway Pump Station Addition.
Change Order No. 5 covers the extra work required to add the SCADA components as requested by the City.

CHANGE ORDER NO. 5
A. INTENT OF CHANGE ORDER
The intent of this change order is to modify the provisions of the contract entered into by the City of Coppell,
Texas and North Texas Municipal Construction, Inc., 5614 Dyer Street, Dallas, Texas 75206, for construction
of the Village Parkway Pump Station Addition, dated January 9, 1996.
B. DESCRIPTION OF CHANGE
The City of Coppell has requested the following additions or modifications to the SCADA System. The
attached proposal from Team Controls more specifically describes the proposed equipment.
a) Adding two (2) pressure point nodes at Coppell Road and Moore Road, utilizing existing telephone
lines and existing equipment where possible.
b) Adding an auto-switch over from the new level transmitter to the existing level transmitters at the
Sandy Lake Road and DeForest Lift Stations. The selection of the level transmitters can be made in
the field or the SCADA program.
e) Adding the Dallas Water Utilities inflow to the new SCADA system by re-installing the existing
modem connection to the Sandy Lake Road Pump Station and re-establishing the signal to the existing
DWU flow indicator and chart recorder at the Village Parkway Pump Station. This work is to be
performed on an hourly basis with a not to exceed amount as indicated in Section C of this change
order.
