review, we received the required initial fee of $1,065.
Review and processing costs associated with this request total $1,065. In addition, because your request will result
in a revision to one FIRM panel, an additional $410 must be submitted. Please forward payment representing the

Please note that we will not issue our determination until this payment is received. Please also note that the initial
fee of $1,065 will only be applied to this request if we receive the required fees within 90 days of the date of the
original letter. After 90 days, the request will be treated as an original submittal and the total review and processing
fees of $1,475 will be required before the requested LOMR will be issued.
