One June 10, 2015, a representative of GEE Consultants, Inc. was authorized to perform
visual observations of the proposed subgrade beneath the multiple box culvert. The box culvert was
located in Southbound Coppell Rd., just North of Bethel Rd.
We observed a layer of larger gravel placed in the bottom of the multiple box culvert
excavation. It was our understanding this gravel layer was about 12 inches thick and placed on
firm soils beneath. No observations or testing of the subgrade beneath the gravel layer was
requested or performed.
Water had collected on top of the gravel layer at numerous locations. Therefore concrete
placement was cancelled on this date.
