Q~ .~ ~,~ ~ ~.- ~ Assistant City Manager/City Engineer City of Coppell ~ 0,0~ ~ Post Office Box 478 J Coppell, Texas 75019 w ~ Re: Village Parkway Pump Station Addition Contract Documents Dear Mr. Griffin: We are enclosing six sets of the Proposal and Bid Schedule, the Standard Form of Agreement along with Change Order No. 1 for the construction of the Village Parkway Pump Station Addition project. Change Order No. 1 deducts $5,000 from the contact amount in order to account for the difference between the bid allowance and the actual proposal received for the proposed pumps and motors. These sets have been executed by North Texas Municipal Construction Company, Inc. Please have all six sets executed on behalf of the City of Coppell at the places indicated, and returned to our office. We will assemble the executed contract documents together with the project specifications, bonds and insurance certificates and distribute three complete sets of the contract documents to you and the contractor at the pre-construction conference for this project. We have reviewed the certificates of insurance and the necessary bonds submitted by the contractor. The Performance, Payment and Maintenance Bonds appear to be in general conformance with the project requirements and the issuing company is licensed in the State of Texas to provide the coverage indicated. The companies providing insurance coverage are also licensed in the State of Texas. However, the certificate of insurance submitted does not include the additional insured coverage for the engineer as required by Item 1.26 of the General Provisions. We have discussed the insurance requirements and policy endorsements with North Texas Municipal Construction Company, Inc., and it is our understanding they are working to make the necessary corrections and re-submit the certificates of insurance.
