Dear Mr. McLean:
The work to which this Certificate applies has been inspected by
authorized representatives of the Engineer and that work is
hereby declared to be substantially complete in accordance with
the Contract Documents as of August 25, 1989.
A tentative list of items to be completed or corrected is
attached hereto. This list was prepared based upon a preliminary
walk-thru made by our Resident Project Repre ttive, City
Personnel and McLean Construction, Inc.fJQl~J~u~2~ 1989. This
list may not be all inclusive and the lude an item'
on it does not alter the responsibility of the Contractor to
complete all 'the work in accordance with the Contract Documents.
The items on the tentative list shall be completed or corrected
by the Contractor prior to requesting a final walk-thru on the
project.
This Certificate does not constitute an acceptance of work not in
accordance with the Contract Documents, nor is it a release of
Contractor's obligation to complete the work in accordance with
the Contract Documents.

Remove 24"x24" tee and other fittings in vicinity of Sta.
0+00, Line "A".
Provide City with 2 valve operating wrenches.
Provide documentation that bacteriological samples have
passed tests.
Install gravel bedding in bottom of 3/4" corporation stop
near Sta. 0+00, Line "A".
Paint relocated fire hydrant at Sta. 7+32, Line "A".
Clean out silt build up in Grapevine Creek, as discussed.
Replace yellow traffic buttons on Southwestern Blvd., west
of Belt Line/Denton Tap intersection.
Repair or replace electrical pull box, northwest corner
of Denton Tap/Belt Line/Southwestern intersection.
First inlet east of Gulf Station (Sta. 43+20 ±); need to
clean debris from bottom of inlet.
Touch up paint at signal pole, NW and SE corner Denton
Tap/Belt Line intersection.
First ARV east of Gulf Station (Sta. 44+00), remove gravel
in bottom in order to operate hand wheel on valve.
Provide City with 2 hand wheel operators for air relief
valve.
Creek bottom, Sta. 80+25, needs to be cleaned up, shaped up
and dressed up.
ARV, Sta 97+70, is leaking. Need to repair and remove water
from vault.
ARV, Sta. 110+04, need to pump vault clear of water, make
sure valves are open. Repair ball valve handle to operate
properly.
ARV, Sta. 123+75, needs handwheel on valve to be installed.
ARV, Sta. 159+20, repair leak on corporation stop.
Line D, Moore Road, south of Carter, need to provide valve
nut extension and make sure tapping valve operates properly.

Rout and seal concrete, Moore Road south of Carter, where
tapping valve installed.
Prune deadwood and paint trees in media~'along MacArthur as
required and discussed.
Remove all dead grass (solid sod) and replace with new sod,
fertilize and water as required.
Repair any sprinkler heads that were determined to have been
disturbed by Contractor, if any. Locate sprinkler heads,
using contract plans, mark on curb with paint for Parks
Department to repair next week.
Traffic buttons north of Sandy Lake need to be removed and
replaced properly with correct epoxy.
On Line "C" - Parkway Blvd. valves on southeast and
southwest corner of intersection of Parkway Blvd. and Denton
Tap needs to have valve extensions provided.
Backfill HMACP shoulder at Belt Line/Denton Tap northwest
radius.
Replace 4" yellow buttons, with proper epoxy, on Belt Line
east of Gulf Station..
At Starleaf crossover, repair random cracks, south side of
median.
At Mapleleaf, repair curb at south nose of median opening.
Repair curb "ear" at redwood header; correct irregular curb
dimensions.
Curb at north nose, Mapleleaf crossover, cracking where
attached to existing concrete pavement needs to be addressed.
Epoxy 4" diameter lane buttons north of Sandy Lake along
MacArthur Blvd.; northbound lanes.
Skin patch concrete hole at start of reverse curve station
of northbound lane, left turn lane.
Sweep and clean dirt from brick wheel chair ramps at Village
Parkway and MacArthur Blvd.
Rout and seal joints on Moore Road, south of Carter Drive.
Replace damaged traffic signal pull box at Sandy Lake and
Denton Tap.
Repaint/touch up signal poles at Denton Tap/Belt Line.
Replace 6-8" diameter single reflective yellow traffic
buttons on Southwestern Blvd.
37. Hand rake dirt away from traffic controller at Belt Line and
Denton Tap Road.
Following are separate punch list items prepared by City of
Coppell representatives who made the walk-through also. Some may
be duplicates of the above list.
We have not received written comments from all departments,
however it is our understanding that there may be a punch list
from the City of Coppell Parks Department forthcoming. When we
receive that list, we will forward a copy to you.
When all items on all lists have been completed or corrected,
please notify this office, in writing, in order that a final
walk-through may be made. You may begin processing of final
close-out documents as per the contra&t specifications, Section
01700. We will not request City Council approval and acceptance
of the project until all punch list items have been addressed or
corrected and close-out documents have been submitted.
