Attached is the stormwater permit form required by TCEQ. Feel free to make any modifications you feel necessary. Once it's filled out and signed, you can keep the original and send a
scanned copy to Scott Morris with the City of Dallas (scott.morris@dallascityhall.com), Archer Western, and us.

We typically require the contractor to file these on our behalf. Is that not included in the contract on this project?
