The undersigned arrived on-site, as requested, to perform floor flatness and levelness testing for
3 panels in Slab on Grade, Pour 3. Please see attached sketch and graphs.
The testing was performed using a D-meter®. Based on the results of our testing, Runs 1-4 had
a combined FF of 68.0 and FL of 29.4, Runs 5-8 had a combined FF of 64.4 and FL 69.3, Runs
9-12 had a combined FF of 64.1 and FL of 51.0.
