The undersigned arrived on site, as requested, to observe:
1: The compaction of soils for Sidewalk and Transformer Pad.
Utilizing the nuclear test method to check the compaction of soils; test results indicated that the
compacted material, at the areas and elevations tested, met or exceeded the project
requirements of 95% of the maximum dry density and moisture content above OMC as
obtained in our laboratory using the ASTM D 698-12 Method A Standard.
Locations and elevations of all tests are based on stakeout provided by others. We cannot be
responsible for structures located off of the observed engineered pad, misaligned utilities or
stakeout errors causing uncontrolled fill to be placed in structural areas.
2: The placement of 123 cubic yards of 3600 psi concrete for Pavement.
3: The placement of 19 cubic yards of 3000 psi concrete for Sidewalk.
The required test limits were determined to be a slump of 3-5 inches, air content of 3-6 %,
batch to placement time of 90 minutes, and concrete temperature range of 50-95 degrees
Fahrenheit.
Concrete observed on this date appeared to be placed in compliance with project specifications
with regard to temperature, slump, air content, and batch to placement time.
Two sets of five laboratory cured concrete cylinders were cast for compressive strength testing
in our laboratory.
