On September 20, 2016 reinforcing steel was observed at the following locations:
1) E. Bound Bethel Rd. Sidewalk, from NE. Comer of Park Rd. to Approximately 180'E. of Park Rd.
*There were no discrepancies.
*Note: Arrived on site at 10:45 AM and one-third (1/3) of rebar at W. end was already covered with concrete. No
discrepancies observed in eastern two -third (2/3).

Results
On September 21, 2016 reinforcing steel was observed at the following locations:
1) E. Bound Sidewalk Sections from Hearthstone Ln. E. to about 350'E. of Park St.
The following discrepancies were noted and called to the attention of a representative of Axis Construction.
1) Ponding water, wet subgrade and mud covered rebar at HCP ramp located at SW. comer of Park and Bethel Rd. This
was due to sprinkler run off from Benson Ln.
2) Dowel rods missing caps and not tied at 2 expansion joints.
3) Wet subgrade at about 350' feet E. of Park St. due to adjacent sprinklers running excessively at night. This section
of sidewalk was not placed on this date
The above discrepancies were corrected as noted.
Note: Discrepancy No. I required the placement of dry lime at the SW. comer of Park and Bethel Rd., however, due to
excessively moist subgrade adjacent to the HCP landing and the lime not solving the soft conditions, construction joints
were utilized on each side. The complete ramp was not concreted on this date. Photos of these conditions can be provided
on request.

1) Sidewalk from Penfolds to about 120'W. of Penfolds and HCP Ramp at SW. Comer of Park and Bethel Rd.
There were no discrepancies.
Note: This included satisfactorily drying out the soft subgrade noted on September 21, 2016 with lime at the HCP ramp
at SE. comer of Park and Bethel Rd. as well as the sidewalk subgrade about 300'E. of Park
