Charley's Concrete
Tomorrow's Concrete of Choice
February 2, 2017
To: Northpoint Construction
Project: Fairfield Inn; Coppell, TX REVISED
The following mixture(s) are submitted in accordance with ACI 318 Chapter 5, proportioning on the basis of field
experience or trial mixtures;
Mixture Code Description Intended Use
4703 5SK/3000PSI SOG
5172 5.5SK/3600PS1 W/AIR Site Paving
5632 6SK/4000PSI W/AIR .Approaches
Charley's Concrete guarantee of strength per ASTM C94 is provided all field samples are in accordance with
ASTM C172 and ASTM C31 and laboratory testing complies with ASTM C39 and ASTM C1077. Charley's
Concrete will guarantee the strength performance of the concrete mixtures submitted when placed with ambient
temperatures between 40 `F and 100 `F provided plastic properties are not exceeded and concrete is placed within 90
minutes of batch time. This temperature and time range does not supersede project specifications. All concrete
should be placed and cured in strict accordance with ACI guidelines for hot and cold weather concreting and may
require the addition of ice during hot weather placements (at the customer's request). During cold weather
placements all concrete surfaces should be protected for a minimum of 24 hours to allow concrete to attain a
compressive strength of 500 psi.
ACI 304.2R "Placing Concrete by Pumping Methods" states that the inside diameter of the hose should be a
minimum of 3 times the maximum aggregate size in the mixture. It is recommended to minimize or eliminate the
free fall of concrete to prevent segregation and to avoid the loss of air. Boom configurations may require
adjustments to the concrete mixture.
* Due to availability of materials namely natural concrete sand and fly ash, it may be necessary during the course of
this project to substitute a source of material. Substitutions will be in complete compliance with project
specifications and should not affect mixture characteristics or performance.
We appreciate the opportunity to be of service to your company. Please forward any engineer comments and
approvals to our office. We also request you add Charley's Concrete to the distribution list for all laboratory
test results for review and to monitor performance and compliance with project requirements.
If you have any questions or comments regarding this matter feel free to contact our office at your convenience.

Sikament 686

Sikament°
686
High Range and
Mid Range Water Reducing Admixture
Description
Sikament 686 is a high range water reducing admixture utilizing Sika's Visco-
Crete Technology. It's unique formulation is based on polycarboxylate
technology.
Sikament 686 meets the requirements for ASTM C-494 Types A and F.
Applications
Sikament 686 is recommended for use in the production of all high strength
concrete products, whenever high plasticity and increased early and ultimate
strengths are desired.
The superplasticizing action of Sikament 686 provides excellent workability at
low water cement ratios.
Advantages
High Range Water Reducing Applications: When used as a high range wa-
ter reducing admixture, water reduction up to 30% can be obtained. The su-
perplasticizing action allows for the production of high slump flowing concrete
with excellent workability that can be placed with minimum vibration even at
a low water/cement ratio. The dispersing action of Sikament 686 maximizes
cement hydration efficiency and improves concrete's early and long term com-
pressive strengths.
How to Use
Dosage
Mid Range Water Reducing Applications: At a lower dosage, Sikament 686
can be used as a cost effective mid range water reducing admixture or simply
as a water reducing admixture for production of conventional slump concrete.
When used as a mid range water reducing admixture, water reduction up to
15% can be obtained. This application is ideal for use with lean, harsh con-
crete mixes or concrete containing flyash. Sikament 686 will improve
workability and finishability.
The combined water reducing and superplasticizing action provide the follow-
ing benefits:
■ Higher early and ultimate strengths for cost effective high strength con-
crete and earlier structural use of concrete.
■ Higher early strengths allow faster demolding and more efficient use of
forms to precast producers.
■ Increased slump improves workability and reduces labor costs.
■ Full flow action aids in pumping and reduces need for vibration.
■ Greater concrete density reduces permeability and increases durability.
Sikament 686 does not contain calcium chloride or any other intentionally
added chlorides and will not initiate or contribute to corrosion on steel rein-
forcement present in the concrete.
Dosage rates will vary depending on the material used, ambient conditions
and the requirements of a specific project.
For general concreting applications, Sika recommends a dosage rate between
3-12 fl. oz. /100 lbs. (195-780 ml/100 kg) cementitious materials. If maximum
water reduction is required, dosage up to 18 fl.oz./100 lbs. (1170 ml/100 kg) of
cementitious materials may be used. In this case, delayed setting times may
occur. Dosage rates outside the recommended range may be used where
specialized materials such as microsilica are specified, extreme ambient
conditions are encountered or unusual project conditions require special con-
sideration. In this case please contact your local regional Sika office or Sika
technical service department at 1-800-933-7452 for further information.
Mixing For best plasticizing results, Sikament 686 should be added directly
to freshly mixed concrete in the concrete mixer at the end of the batching
cycle.
Sikament 686 may also be dispensed as an integral material during the regu-
lar admixture batching cycle, or into freshly mixed concrete in a Ready -Mix
truck at the concrete plant or job site.
To optimize the superplasticizing effect, Sika recommends that the combined
materials be mixed for 80-100 revolutions, either in the concrete mixer or in
the Ready -Mix truck.
Combination with other Admixtures: Sikament 686 is highly effective as a
single admixture or in combination with other admixtures in the Sika System.
If used in combination with certain Sikament high range water reducers, it
may affect the plastic properties of fresh concrete. Please contact your local
regional office or technical service department at 1-800-933-7452 for further
information.

Sika Air
Sika° Air
Air Entraining Admixture
Description Sika Air admixture is an aqueous solution of organic materials.
Sika Air meets the requirements of ASTM C-260 for air entraining admixtures.
Applications Sika Air is recommended for use whenever air entrained concrete is desired.
Ready -mix, precast and block producers can achieve predictable and uniform
entrained air contents in concrete, even where harsh lean mixes are used or
fly -ash is added to the concrete.
Advantages Durability:
■ Air entrainment is recognized as the most effective prevention against con-
crete scaling in exposed environments. Air entrained concrete delivers par-
ticular benefits in the form of increased concrete durability. This is important
in colder climates where frost and freeze -thaw cycles can cause scaling and
damage to the concrete surface.
■ Air entraining agents help to prevent scaling by creating microscopic air
voids that water trapped in the concrete can expand into when the concrete
freezes, thus preventing cracks caused by the natural expansion. Entrained
air voids in the concrete will also increase durability in harsh environments
where concrete is exposed to deicing salts, marine salts and sulfates.
Workability and Placeability:
■ Workability and placeability are also improved by the lubricating action of the
microscopic bubbles in the concrete. Concrete will flow better, and bleeding
and shrinkage will be reduced because less water is needed to obtain the
desired workability.
How to Use
Dosage Dosage rates for Sika Air will typically fall between 0.5 and 3 fl. oz. per 100
lbs. (32 - 195 ml/100 kg) of cementitious to entrain between 4 and 6 percent
air. Higher air contents may be obtained by increasing the dosage rate.
Dosage rates will vary depending on the air content required for a particular
project. Typically air contents will be specified in the range of 4 to 8 percent by
volume.
Other factors that may affect the amount of air entrained into the concrete
including total cementitious content, type of pozzolanic materials, sand grada-
tion, salt/clay in aggregates, temperature and water content. The use of fly
ash, particularly high LOI fly ash, can result in a higher dosage of air entrain-
ment. Sika recommends that trial mixes be performed whenever material or
any other changes are made that may affect the amount of entrained air.
In mixes requiring a lower or higher amount dosage rate, please contact your
local regional Sika office or Sika technical service department at 1-800-933-
7452 for further information.

SECTION 3 - HAZARDS IDENTIFICATION
Emergency Overview:
Short term exposure to wet concrete is not likely to cause an immediate hazard. However, exposure of
sufficient duration to wet concrete can cause serious, potentially irreversible tissue (skin or eye) damage in the
form of chemical (caustic) burns. The same type of tissue damage can occur if wet or moist areas of the body
are exposed for sufficient duration to the dry ingredients in unhardened concrete.
Routes of Exposure:
Eve Contact
Exposure to airborne dust during the handling or mixing of the dry ingredients in Ready -Mix concrete may
cause immediate or delayed irritation or inflammation.
Eye contact by splashes of wet concrete may cause effects ranging from moderate eye irritation to chemical
burns and blindness. Such exposures require immediate first aid (see Section 4) and medical attention to
prevent significant damage to the eye.
Skin Contact
Discomfort or pain cannot be relied upon to alert a person to a hazardous skin exposure. Consequently, the
only effective means of avoiding skin injury or illness involves minimizing skin contact, particularly contact
with wet concrete. Exposed persons may not feel discomfort until hours after the exposure has ended and
significant injury has occurred.
Exposure during the handling or mixing of the dry ingredients in Ready -Mix concrete may cause drying of
the skin with consequent mild irritation or more significant effects attributable to aggravation of other
conditions. Exposure to wet concrete may cause more severe skin effects including thickening, cracking or
fissuring of the skin. Prolonged exposure can cause severe skin damage in the form of (caustic) chemical
burns.
Some individuals may exhibit an allergic response upon exposure to wet concrete. The response may
appear in a variety of forms ranging from a mild rash to severe skin ulcers. Persons already sensitized may
react to their first contact with the product. Other persons may first experience this effect after years of
contact with wet unhardened concrete products.
Ingestion
Although inadvertent ingestion of small quantities of wet concrete or its dry ingredients are not known to be
harmful, accidental ingestion of larger quantities can be harmful and requires immediate medical attention.
Inhalation
The ingredients in Ready -Mix concrete contain crystalline silica. Exposure to these ingredients in excess of
the applicable TLV or PEL (see Section 2) may cause or aggravate other lung conditions.
Exposure to the dry ingredients in Ready -Mix concrete may cause irritation to the moist mucous membranes
of the nose, throat, and upper respiratory system
Medical Conditions which May be Aggravated by Inhalation or Dermal Exposure:
Pre-existing upper respiratory and lung diseases by exposure to the dry ingredients.
Persons with unusual (hyper) sensitivity to chemicals, dusts, and metallic compounds may experience adverse
reactions to wet concrete.
Carcinogenic Potential:
Ready -Mix concrete is not listed as a carcinogen by NTP, OSHA, or IARC. It may, however, contain trace
amounts of substances listed as carcinogens by these organizations including but not limited to: crystalline
silica, hexavalent chromium, lead compounds, mercury compounds, nickel compounds, and possibly other
chemicals which may result in exposures which require the following warming pursuant to California Proposition
65:
WARNING: This product contains chemicals known to the State of California to cause cancer, birth
defects, or other reproductive harm.

Eyes:
Immediately flush eyes thoroughly with water. Continue flushing eye for at least 15 minutes, including under
lids, to remove all particles. Call physician immediately.
Skin:
Wash skin with cool water and pH -neutral soap or a mild detergent intended for use on skin. Seek medical
treatment in all cases of prolonged exposure to wet concrete, liquids from wet concrete products, or prolonged
wet skin exposure to the dry ingredients in Ready -Mix concrete.
Ingestion:
Do not induce vomiting. If conscious, have the victim drink plenty of water and call a physician immediately.
Inhalation of Airborne Dust:
Remove to fresh air. Seek medical help if coughing and other symptoms do not subside. (Inhalation of gross
amounts of the dry ingredients in Ready -Mix concrete requires immediate medical attention.)

SECTION 6 - ACCIDENTAL RELEASE MEASURES
Collect dry material using a scoop. Avoid actions that cause dust to become airborne. Avoid inhalation of dust and
contact with skin.
Scrape up wet material and place in an appropriate container. Allow the wet concrete to "harden" before disposal.
Do not attempt to wash wet concrete down sewers or storm drains.
Wear appropriate personal protective equipment as described in Section 8.
Dispose of waste material according to local, state and federal regulations.
SECTION 7 - HANDLING & STORAGE
Normal temperatures and pressures do not affect the material.
Promptly remove dusty clothing or clothing which is wet with concrete and launder before reuse. Wash thoroughly
after exposure to dust or wet concrete mixtures.
SECTION 8 - EXPOSURE CONTROLS/PERSONAL PROTECTION
Eve Protection:
When engaged in activities where wet concrete or its dry ingredients could contact the eye, wear safety glasses
with side shields or goggles. In extremely dusty environments and unpredictable environments, wear unvented
or indirectly vented goggles to avoid eye irritation or injury. Contact lenses should not be worn when working
with wet concrete or its dry ingredients.
MSDS 0008
SECTION 8 - EXPOSURE CONTROL/PERSONAL PROTECTION (continued)
Skin Protection:
Prevention is essential to avoiding potentially severe skin injury. Avoid contact with unhardened (wet) concrete
products or its dry ingredients. If contact occurs, promptly wash affected area with soap and water. Where
prolonged exposure to unhardened concrete products might occur, wear impervious clothing and gloves to
eliminate skin contact. Where required, wear boots that are impervious to water to eliminate foot and ankle
exposure.
Do not rely on barrier creams; barrier creams should not be used in place of gloves.
Periodically wash areas contacted by wet cement or its dry ingredients with a pH neutral soap and water. Wash
again at the end of the work. If irritation occurs, immediately wash the affected area and seek treatment. If
clothing becomes saturated with wet concrete, it should be removed and replaced with clean dry clothing.
Respiratory Protection:
Avoid actions that cause dust exposure to occur. Use local or general ventilation to control exposures below
applicable exposure limits.
NIOSH or MSHA approved particulate filter respirators should be used in the context of respiratory protection
program meeting the requirements of the OSHA respiratory protection standard [29 CFR 1910.134] to control
exposures when ventilation or other controls are inadequate or discomfort or irritation is experienced.
Respirator and/or filter cartridge selection should be based on American National Standards Institute (ANSI)
Standards Z88.2 Practices for Respiratory Protection.
Ventilation:
Use local exhaust or general dilution ventilation to control exposure within applicable limits.

SECTION 16 - OTHER INFORMATION
Other Important Information:
Ready Mix concrete should only be used by knowledgeable persons. A key to using the product safely requires
the user to recognize that portland cement chemically reacts with water, and that some of the intermediate
products of this reaction (that is, those present while a concrete product is "setting") pose a far more severe
hazard than does portland cement itself.
While the information provided in this material safety data sheet is believed to provide a useful summary of the
hazards of unhardened (wet) Ready -Mix concrete as it is commonly used, the sheet cannot anticipate and
provide all of the information that might be needed in every situation. Inexperienced product users should
obtain proper training before using this product.
In particular, the data furnished in this sheet does not address hazards that may be posed by materials other
than natural sands and gravels mixed with portland cement and flyash to produce Ready -Mix concrete products.
Users should review other relevant material safety data sheets before working with this portland cement
concrete.
