Describe and Attach any Pertinent Documents: (Police Reports, statements, receipts)
At 3:30pm when we pulled back our pipes. at the head of the rig black & white pipe and fiber came out as well. We
were boring at 10’deep in that area. We check level 3 and Zayo’s hand hole to see for any damage to the fiber by then
a level 3 truck pulled up asking if we hit anything. We showed him what was damage. We walk the work area with him
taking pictures and showing him our path. There was only one area we’re our path cross. And we pot holed it, we
found the fiber 4’ deep and we still had it open. They tried to locate the pipe but had no luck locating it. Due to it not
being we’re it was supposed to be at. They took pictures talk to Shane Abbott about the situation. Came to agreement
that we would let them use one of our pipe to restore the service. And they would replace our third 1’ ¼ green pipe
asap. And everything will be figured out later. I stayed till they found there fiber conduit and took pictures how far away
we were at both ends. They had no way of finding out were there fiber was running and that was the only fast solution
to the problem.
