Additional Data Required to Support a Request
for a Flood Insurance Study (FIS)/
Flood Insurance Rate Map (FIRM) Revision

The data listed below must be provided before we can process your request for
an FIS/FIRM revision.
1. Evidence of compliance with National Flood Insurance Program (NFIP)
Regulation Part 65.7(b)(1), which requires that when a floodway
change is proposed, a copy of the public notice distributed by the
comm~unity stating the community's intent to revise the floodway must
be submitted, or a statement by the community that it has notified
all affected property owners and affected adjacent jurisdictions.
2. Evidence of compliance with NFIP Regulation 65.6(a)(12), which
requires documentation describing how the flood-carrying capacity
within the channelized portion of Grapevine Creek is maintained.
This documentation, which may be in the form of a written statement
from the CEO, an ordinance, or another legislative action, shall
describe the nature of maintenance activities to be performed, the
frequency with which they will be performed, and the title of the
local official who will be responsible for assuring that the
maintenance activities are accomplished.
3. Technical assessment which addresses the probable stability of the
site during the occurrence of the 100-year flood. This is required
to ensure that the channelization will function as designed. Of
particular interest are locations where channel velocities are
greater than 5 feet per second and erosion protection methods such as
riprap or channel lining are not being used.
