Gentlemen:
The City of Coppell, at their regular meeting on May 23, 1995, accepted your proposal for construction of the
Village Parkway Ground Storage Reservoir No. 2 project in the amount of $2,809,305.00.
We are enclosing six (6) copies of the following documents:
· Proposal · Affidavit of Safety Record
· Bid Schedule · Standard Form of Agreement
· Bid Summary · Performance Bond
· Bidder Qualification · Payment Bond
· Bid Affidavit
Please execute and return all six copies of each document to our office. Also, SC~ 1.58 of the Supplementary
Conditions of the Agreement requires the development of a "separated contract" which separates charges for
materials actually incorporated into the project from materials consumed during the construction of the contract
and labor charges. Please furnish an itemized breakdown (per bid item) of:
1. Materials incorporated into the project.
2. Labor, equipment, supervision and materials not incorporated into the project
After the agreement has been signed by the City, two complete copies of the contract documents and
specifications will be furnished to you. The project Maintenance Bond shall be completed and executed prior
to final payment on the project.

Also, please furnish the Certificates of Insurance as required by Item 1.26 - Insurance of the General
Provisions, and Item 1.26.6 - Insurance of the City of Coppell Supplementary Conditions to the General
Provisions of the specifications; and the Trench Safety Plan as required by SC. 15 - Trench Safety_ Systems of
the Special Conditions of the standard specifications. We are enclosing one copy of the specification for your
reference in preparing the necessary documents.
