Saw cut and pemove the existing conePete pavement and conepete
median at the intepsection of Southwestepn Blvd. and Fpeepopt
Papkway. Excavate to a depth of 12 inches below top of exist-
ing conePete. Remove all spoils Pocks, bollapds, and GTE Util-
ity vault fpom job site. Backfill vault apea with suitable
matepial. Place 6 inches of Cement Tpeated Base. Pave with 8
inches of 3500 psi eoncpete~ peinfopeed with ~3 pebap on 2N
inch eenteps~ dowelled into adjacent conepete.

All material is guaranteed to be as specified, and the above work to be performed in accordance with
the drawings and specifications submitted for above work and completed in a substantial workman-
like manner for the sum of Seven Thousand 8 no/100 Dollars ($7,000.00)

TERMS AND CONDITIONS
1. Any deviation from the specifications or modification of the terms of this contract and any extra or incidental
work shall be set forth in writing and signed by both parties thereto.
2. Company will provide PUblic Liability and Property Damage Insurance.
3. If, in the opinion of Company, your credit becomes unsatisfactory before commencement or at any time
during the course ofthework, adequatesecurity shall be furnished by you to Company upon its request, or
Company shall have the right to discontinue work treat the contract as having been breached by you, and
bring suit for the completed work and for its damages by reason of your having breached this contract.
4. There shall be no liability on the part of the Company for damages resulting to utilities or other facilities or
objects buried beneath the area of construction.
