REPORT OF INSPECTION
On September 7, 1995 representatives of Roger O'Brten, GDA, CEI and Maxim
Technologies met to review the roofing of the above referenced project.
The following items were addressed:
1. Safety
a. OSHA requirements
b. No Smoking on the roof.
c. Safety meetings on Tuesday or Wednesday's
d. Roof access will be a stair tower on the south side of the building.
2. Daily trash removal will be required.
3. Material Storage
a. Do not overload roof.
b. All material to be raised and protected from the elements.
4. Scope of Work
a
Inspect parapet nailers for compliance.
Panel seams to be caulked prior to roof placement.
Wood blocking is to be flush with the deck.
Additional blocking at scuppers. Scupper to be 2" above the drain.
No cants or tapered will be used at wall perimeter.
Membrane corners will be fabricated in-field.
Deck inspection scheduled for 9-11-95

t. Firestone EPDM membrane to be placed, per manufacturers
specifications.
J. 1-90 wind up-lift pattern to be used during insulation placement.
5. Maxim will be responsible for vtual examination of roof placement and
the reporting of the same. 9 - 2 hour spot checks, 1 4 hour punch list
inspection and 1 3 hour final inspection will be performed during the
estimated 5 weeks of roof placement.
6. A hand written field report will be left on-site with Roger O'Brien
representatives at the end of each inspection. Copies of the completed
report will be faxed to GDA, CEI and Roger O'Brten the following day of
inspection. Any deficiencies will be reported immediately to the roofing
foreman, Roger O'Brien and GDA.
