This Punch List Observation Report was conducted on Monday, November 18, 1996 to form the
Initial Review of the construction.

The following is a Review by Phillips Swager Assodates (PSA). Consultant Punch List is
attached behind this report.
Room 101 - Lobby
1. New door not installed.
2. Paint interior window frame.
3. Clean electrical plates.
4. Touch-up wall paint by leak.
5. Base missing at interior edge of interior window.
6. At E109D - paint hinges grey.
7. Fill broken CMU.
8. Paint back of wall of EWC and aH around.
9. Replace water damaged ce'ding tile.
Room 101 - Back Door Area
10. Block needs to be painted on exterior.
1 I. Seal at door head.
12. Seal'at hinge side of jamb.
13. Paint door.
14. Clean stains in carpet.
15. Patch holes by EWC.
16. Close gap at base of jamb (back door).
Room 102 - Office
17. Paint doorjamb.
18. Remove tape from light fixtures.
19. Secure plates.
20. Touch-up door paint.
21. Adjust door to open/close without scraping floor.
Room 103-
22. Patch holes in wall.
23. Fix split door.
24. Secure door hardware.
Room 104-
25. Touch-up door jamb.
26. Touch-up paint at deadbolt.
27. Cover exposed wire.
28. Hole in ceiling at diffuser.
29. Remove hook, chain, 2 x 4 - paint and patch ce'ding.
30. Touch-up paint on walls/ceilings.
Room 105 - Men
31. Repair hole in door by lock.
32. Paint CMU.
33. Add missing color tile in Men's room wainsot.
34. Fix broken tile at wall plate.
35. Caulk at hand bars.
Room 106-
36. Could not get into room because door was not operational.
Room 107 - Kitchen
37. Fill hole in ceiling by new pendant fixtures.
38. Need caulking at backsplash and counter wall connection.
39. Caulk at base cabinet next to wall.

40. Touch-up paint on walls and ceilings.
41. Base missing at doorjamb by CMU.
42. Remove return/supply grilles and clean. Re-install.
43. Remove return/supply grilles and clean. Re-install.
44. Clean wood trim edge on cabinets.
45. Install hardware in cabinets.
46. Patch/paint hole in wall.
Room 110 & Vestibule
47. Paint wall and touch-up paint.
48. Clean door hardware.
49. Caulk around mirror.
50. Caulk around paper towel dispenser.
Room 111 - Craft
51. Chipped paint on wall.
52. Seal gap above air grille.
53. Coat of paint on access panel in ceiling at rear door.
54. Patch hole at old laundry area.
55. Cracked and scraped wall.
56. Touch~p paint at ceiling
57. Repair door hardware on rear door so that door is operational without force.
Room 113 - StoraEe
58. Clean rubber base.
59. Touch-up paint.
60. Remove miscellaneous metal accessories and patch/paint.
Room 108 - Electrical
61. Adjust tension on door.
Room 109-
62. Caulk millwork edge.
63. Caulk around base of columns.
64. Paint wall by Electrical conduit.
65. Clean wood trim on millwork.
66. Flanges removed from wall and wall patched and painted.
67. Patch holes in CMU wall.
68. Scrape sealant at wing wall and re-paint.
69. Cut screws, sand down and paint at new conduit installations.
70. Adhere VCT tiles throughout so surface is smooth.
71. Clean CMU block of ceiling paint.
72. Caulk at door.
73. Patch holes in CMU - north and west wall.
74. Hardware for storefi-ont missing.
75. Touch-up paint - especially at wingwall.
76. Trim boards - adhere to CMU wall.
77. Clean storefront frame.
78. Clean grey paint offCMU.
79. Adjust door El07 to close correctly.
80. Paint conduit.
Door 109D -
81. Adjust strike.
82. Make threshold ADA compliant per Addendum.
83. Paint door.
GENERAL NOTES:
1. Tack down rubber base throughout.
2. Touch-up paint throughout.
3. Secure and clean electrical plates.
4. Signs not installed.
5. Touch-up paint at all doors throughout.
6. All touch-up paint to match base paint.
7. Clean building overall.
8. Sealant.

Coppell Senior Center:
1. Existing Air conditioning unit would not run from thermostat check
operation.
2. T-stat for new unit was not wired.
3. Exhaust fan in Mens' Toilet was not switched.
4. Exit sign over exterior door (rm. 111) interferes with traffic through
door. Relocation to beside door is recommended.
5. Many exit signs are crooked.
6. Verify switching of lights in craft room is acceptable to owner (switching
configuration may be existing condition). One light is not switched and
the emergency lamps in the center fixture also are not switched.
7. The emergency fixtures in the multipurpose room are not switched. The
owner should be consulted to determine if they want the fixtures to be on
continuously or switched.
8. One lamp missing in the "D" fixture.
9. C1 fixture and switch missing from elect room.
10. Some kitchen lights not operating.
11. Switch missing in Janitor's closet.
12. Fan controls appear to be lighting dimmers rather than fan speed
controllers.
13. Emergency pull cords, and dome lights were not installed.
14. The switches for the corridor lights were not controlling the lights.
15. Smoke detectors not installed yet.
16. The time clock had an exposed bare wire connection.
17. The telephone board was not installed.
18. Existing wiring by the door into the elec. rm. has not been removed.
