Note: Panels 21, 27, 29, 28, S24, S25, 20, 19, 18, 17 and 16 were
to technician's arrival. HBC was not notified 24 hours prior to cot
placement. Concrete was batched on site and not getting proper mix
prior to being placed in forms. Approximately 10 minutes passed fr(
of batching until placement of concrete into forms for tilt panels.
Superintendent with Butler Construction could not be located in ord~
him of problems. Samples taken from truck discharge; pump hose was
accessible.
