Punchlist Items for Oakbend Addition

The following items have been noted on 5/15/91 and include
State Street:
- Erosion must be removed from all street and alley paving.
- Construction joints and headers need to be sealed to back
of curb.
- Repair MUD ball holes in paving.
- Repair curb as noted. (1)
- Clean up pipe connection to inlet. (2)
- Erosion around headwall and at outfall PT. (3)
- Remove conc. chips from joint and reseal. (4)
- Repair as necessary the alley where the corner of a panel
appers to be damaged. (5)
- Re-fill construction joints (primarily alleys) where sealant
is below 1/8" from surface of pavement.
- Finishing sealing alleys.
- Pour and finish headwall into channel.
- Backfill edge of asphalt transition with non-erosive
material.

State Road Drainage
- Grade/reshape ditch erosion.
- Backfill around all exposed edges of conc. rip-rap.
- Clean dirt & const, debris from both barrells of culvert.
- Remove excess ramneck from joints.
- Address erosion in channel East of State.
- Erosion protection needed at outfall of box culverts.
- Guardrails needed.
Special Notes
House construction in the cul-de-sac near the channel has partially
filled the channel banks changing its designed slope - all fill must
be removed.
Due to the extensive amount of dirt erosion on pavement it cannot
be determined if any ponding exist and will have to be water tested
at a later date.
- Signs on State Road need to be resolved.
