erosion at headwall.
4.Clear trash, debris, mud and dirt from ~ll ,,y- inlets and inlet boxes.
$,Bad connection at RCP and main - grout,
6."Y" inlet not accessible, lock broken.
7.Forms left in inlet boxes.
8.Inlet box holdin~ water, pour in bottom and grout at bottom of box.
9."Y" inlet not accessible - full of mud.
10.Obstruction at RCP and box from the bottom being poured, and leaked into RCP.
