Dear Mr. Griffin:
We have received your request dated June 18, 1996, for a Conditional Letter of Map Revision based on fill for the
property above.
The initial fee of $280 that you have submitted is the minimum charge associated with our review of a request of this
type. You will be notified of the estimated total processing costs ff we anticipate that, due to the extent of the review
required, the costs will exceed $1200. In that situation, our review would be suspended pending our receipt of written
authorization from you to proceed. If your request results in a revision that can be shown on the effective National Flood
Insurance Program map, there will be an additional fee of $410 per revised panel to cover the costs of cartographic
preparation.
We have completed an inventory of the information that you submitted. Before we can begin a detailed review of the
request, we must receive the following items:
· Elevation Information form (copy enclose)
· Summary of Elevations--indi~du~al4-~0t Breakdown form (coPY enclosed)
· Copy of the plat map (w~iflfrecordafion data)
· Commlmi~la~Swledgment f~rm (TPY enclosed)

If the required items are not submitted within 90 days of the date of this letter, any subsequent submittal will be treated
as an original submittal 8nd will be subject to all submittal/payment procedures, including the initial fee.

except initial fee, and any technical questions concerning your request should be directed to our Technical Evaluation
