Remarks: On this date, a representative of Fugro South, Inc. was present on the above mentioned project
to perform the following tests and observations as requested:
Observed cable stressing
1 ) All cables except #3 and #17 on Tennis Court 6 & 8 cut and grouted per tennis court subcontractor and
redi-cables request.
2) Cables ~ and #17 broke at anchors on Tennis Court 6 & 8 (#3 & #17 off northwest comer) not repaired
on 4-13-00.
3) Cable #34 and Tennis COurt #5-7 (off northwest corner) had no anchor. Concrete chipped out and new
anchor set. Cable repaired.
