1. Junction box with forms left in top.
2. From inlet box to junction, we have a 5" belly in R.C.P. and
the first joint looks bad. Wrong lid is on junction.
3. At R.C.P. and junction grout pipe and junction.
4. Mud in inlet boxes, grout boxes.
5. Grout pipes at boxes, grout pipes at junction boxes, main is
full of mud.
6. See attached control layout sheet.

1. SecOnd [ire hydrant valve needs an extension.
2. Third fire hydrant valve needs an extension.
3. Need new me~er can and expose curb stop on 2" meter.that sits
in front of Kuehne & Nabel. A valve stack needs ~o be on. the
curb stop valve.
4. Sewer cleanout at Wrangler & Lakeshore, west side, needs re-
placin9.
5. Fire ~ydrant valve cover on west side of Stadium needs to be
painted p~us the man line valve cover needs to be painted.
6. ~lean valve stack south on Freeport. . ..
Locate manholes from Crestside driv~ through to Beltline a
total of 10 manholes. All manholes need rain pans and chimney
seals.

1. Seal all joints and random cracks including curbs marked in
white.
2. ~o~ding & sealant at west entrance to 1110 Executive.
3. ~treet.name blade missing at Executive and_wrangler. . ....
4. Curb broken at intersection of Executive & Wrangler, painted
white.
5. _All.spal~ed (holes) areas painted white-cut out and repair._
6. Need to check all appreache~ coming out to Wrangler for areas
painted white.
7. eonding in curb line at Enterprise & Wrangler also replace all
~ealant at t~is intersection.
8. Replace broken buttons ~outh of Enterprise painted white.
9. White & yellow buttons. at Wrangler at Freeport have mi~sin~
reflectors-need reglaced.
10. S_~.ree~...blade missing at Wrangler & Freeport.
11. Random cracks & joints at Wrangler & Freepgrt (lots of them.).
