In order for a utility permit request to be approved, three (3)
complete sets of the following three (3) items must be included.
A separate request must be made for each highway in which
facilities are proposed.
1. Notice of Prnnn4ed T stanation - Form 3L023 (Non-Controlled
Access Highway) or Form 1082 (Controlled Access Highway).
A. Form must in the name of the utility agency with at least
one (1) original signature by utility agency employee with
signature authority. Consulting Engineering Firms or
Developers are not acceptable.
B. The front page of the form must be completely filled out.
Page two will be computer generated by Tx=. 'SEE ATTACHED'
will not be accepted as a description of work.
2. Location M&12 - Copy from a county map or MAPSC4 With Work
location .indicated.
3. plans - The first one to three feet from the ROW line is
reserved for aerial facilities. Three to ten feet from the ROW
line is for buried facilities.

A. Schematic drawings(wiring diagrams) are not acceptable plans
for permits.
B. Only include what is located in state ROW. Most permit
requests can be drawn on a single 8.5 x 11 sheet of paper.
Do not submit a complste set of plans for a private
development - copy only the portion showing the service taps
in public right of way.
C. Plans must show right of way, lines, existing /Proposed
pavement, and existing /proposed drainage structures.
Distances must be given from proposed utility facilities to
right of way lines. Distances to the pavement (back of curb)
are secondary.
D. A profile(cross sectional view) for any crossing of a state
maintained highway must be included. The profile must show
depth of bore, pavement is ROW width, and bore & encasement
lengths.
E. Scalable drawings with a arrow are helpful.
F. Traffic control plans are required to approve a lane closure.
If a traffic control plan is not included with the permit
request, then TxDOT considers a lane closure is not
necessary. Barricades and Traffic Control Plans are subject
to Area Engineer Approval.
Utility permit requests are approved in the order received and
generally require one (1) week to process. In order to expedite
the process you may hand deliver the request and we will call you
to pick up your copy when the permit is approved. Under no
circumstances. will verbal approvals be given. Faxed copies will
not be accepted, nor will approved permits be faxed.
