Services: Drilled belled pier observation.
On this date, an HBC Engineering technician arrived at the above referenced jobsite as scheduled for
drilled belled pier observation. See the attached sheet(s) for recorded data.
NOTE: At 12:45 p.m., the pier drilling rig broke down. HBC's technician was requested to be on
standby until the mechanic checked out the pier drilling rig. At 2:00 p.m., the mechanic arrived
and started working on the pier drilling rig. At 2:45 p.m., HBC's technician was informed that the
pier drilling rig was finished for the day and the pier holes were backfilled.

cubic yards was placed using the remaining concrete from the Elevated Slab and Slab Leaveout concrete
placements.
