We have performed an administrative and technical review of this report and Uased on the information provided it appears that your annual report includes all of the required information. Please note that additional inforination or revisions inay be required as a result of an inspection or an audit to determine coinpliance with the approved inunicipal separate storm sewer system (MS4) Storinwater Management Program (SWMP) and the Phase II MS4 General Permit TXR040000. If there are any differences between the approved SWMP and the inforination provided in this annual report or iuture reports, you must submit a i�otice of Change (NOC} form (TCEQ-20392), as required by the Phase II MS4 General Perinit (TXR040000). The annual report is due within 90 days of each reporting year as specified in your Notice of Intent (NOI). The TeYas Commission on Envirorunental Quality (TCE� has an annual report form (TCEQ-205G1) available, and the MS4 operators inust use the template when preparing and submitting subsequent reports (See Part IV.B.2 of the general permit). This will greatly assist us in reviewing the reports and will help to ensure that you include all the required eleinents. If we have asked you to make changes to the annual report during our review process, please submit the updated annual report to your local regional office. Finally, please be aware that discharges of the pollutant(s) of concern to iinpaired water bodies for which there is a total maximum daily load (TMDL) implementation plan are not eligible for coverage under the Phase II MS4 General Permit unless they are consistent with the approved TMDL and the associated implementation plan. Permitted MS4 operators must incorporate the limitations, conditions and requirements applicable to their discharges, including monitoring frequency and reporting required by TCEQ rules, into their SWMP in order to continue to be eligible for coverage under the general permit If the discharge is no longer eligible for coverage under the Phase II MS4 General Permit, then the MS4 operator would be requir�d to apply for aiiu receive aii iridividual TPDES permi�t.
