based on items in the original bid. Any request i~r bid cost increase must t~ of adequate detail for

There is a conflict: in thc field between somc stom~ dn~ns and a GTE duct bank. No field chm~ges
have occurred to this point. Jay Simms, with GTE, is currently investigating the situation. They
have been asked to pothole the duct bank from 121 to the box culvert to verify depths. GTE was
surprised that there were conflicts. An initial contact with GTE requesting approximate line
locations and informing them of the job occurred in October 1998, and a formal pl:ms .'rod
specifications submittal to GTE occurred in February 1999.

3. In regards to storm drain construction, protection around inlets in progress is needed.
The 12" waterline is in place, except for tie-in at the south end by I O'- 12 ', as inilially planned. The
line has been pressure tested and is currently being flushed. Samples will be taken after flushing.
Tic-ins will occur next form the Mansions to the south. The City water department should be
inw>lved in valve operation,al ~h3 Manelion3 vault. Shut down notifications must occur in all
siluations. One tie per day i.s anticipated by Ed Bell.

It is requested by the City that 'all fire hydrants tenrain operable throughout construction. Ed Bell
has complied and a.~isurcs that they are all usable. Contact with the fire department should be made
bcl'orc bagging fire hydrants.
6. Waterline densities are all in check with the exception of some retakes.
7. Paving operations on the east/west portion arc on hold until the (;'FIE'; conflicts are resolved.
8. There are no known property owner issues at this time.
