These orientations are project specific. b.Work will be in accordance with the Subcontractor 's and General Contractor's safety programs, along with OSHA requirements . c.Subcontractor agrees to provide all required safety equipment, including, but not limited to, safety glasses, hard hats, yellow or orange reflective vests, and gloves. d.All personnel onsite are to wear long pants, sleeved shirts, and proper footwear. e.Subcontractor agrees to report any injuries or near miss accidents to the General Contractor in a timely manner. f.Subcontractor agrees to submit the following in a timely manner: i.Daily Reports ii.Safety Program iii.Hazard Communication Program and Safety Data Sheets (SDS) iv.Accident/Injury Reports(s) v.Any other documentation required by the the Subcontractor's or General Contractor's safety programs g.Subcontractor will provide a Competent Safety Person and implement any necessary safety measures (barricades, spotters, signage, flagging, fire watch, lockout/tagout, etc.) necessary to perform this scope of work. h.Subcontractor will provide an English speaking Foreman onsite during the duration of their scope of work. i.The Subcontractor agrees to the comply with the Owner required background check and badging requirements. Any related cost is the responsibility of the Subcontractor. j.Subcontractor will provide drinking water for it's personnel. 002 Change Orders a.Field staff, including the Superintendent, cannot order or sign for additional scope items resulting in changes in the contract amount. b.Only the Project Manager and Senior Project Manager are authorized to approve scope changes. This includes time and material tickets. Change orders or extra work tickets must have a Joeris change order signed and approved prior to billing. Subcontract Agreement Date: Vendor No.: NORT28 Cost Code: 33-1000-00 S Project # 20029-0 - North Lake College Construction Sciences Building Contract Number: 001 c.Change orders that appear for the first time on pay applications will be rejected and the pay application with be required to be revised prior to approval. d.It is the sole responsibility of the Subcontractor to notify the General Contractor of any change or potential change in cost immediately upon discovery. e.All time and material tickets will have a specified scope of work and must include a not to exceed amount. f.Change order work not approved by the Project Manager or Senior Project Manager in writing will be not be legitimized by the General Contractor. g.Subcontractor agrees to submit an RFI of all changes to the Project Documents for record. h.Overtime requested by the General Contractor must be documented with daily time sheets to be submitted on a weekly basis and approved by the Project Manager or Senior Project Manager. 003 Pay Applications a.Subcontractor agrees to submit pay applications by the 20th of each month. b.Pay Applications to be submitted as outlined in the attached documents . c.Pay Applications must included the required documents as outlined in the attached documents and a Schedule of Values . d.Subcontractor agrees to the Pay Application submission process per the attached documents . 004 Schedule a.Subcontractor will comply with the requirements of the Schedule for the project, including providing input necessary to compile the Schedule. b.Weekly schedule updates and commitments will be completed during the Weekly Progress Status Meeting . c.The Subcontractor agrees to furnish crews necessary to meet the requirements of the project schedule and Substantial d.Completion Date. Failure to do so will warrant the issuance of a Failure to Comply Notice. Further failure will result in the e.Subcontractor being supplemented. All direct and indirect costs incurred in supplementing a scope of work will be deducted from the Subcontractor's contract. f.Subcontractor will mobilize in accordance the project Schedule. g.Overtime required to meet schedule commitments will be the responsibilty of the Subcontractor. 005 Weekly Progress Status Meeting a.Weekly Progress Status Meetings will be held at the jobsite and conducted by the Contractor . b.Subcontractor’s Superintendent/Foreman and Project Manager attendance is mandatory. c.Weekly schedule updates including pull planning sessions will occur during the Weekly Progress Status Meeting . 006 Contract Documents a.Subcontractor is responsible for their scope of work as outlined in the Contract Documents. b.Contract Documents include, but are not limited to, the plans, specifications, addenda, and changes issued by the Architect . 007 Samples, Submittals, and Shop Drawings a.Subcontractor will furnish, at its cost, all samples, mock ups, and shop drawings for submission to the Architect in a timely manner to allow for approval and to prevent any delay to the Schedule. Subcontractor will remove mock up when requested. b.All submittals must be in accordance to the Project Documents and manufacturer's installation instructions. Any potential rework associated with non-compliant installations is the sole responsibility of the Subcontractors. c.Subcontractor maintains that all its products and materials are in accordance with the Project Drawings and Specifications. d.Any substitutions must be submitted in writing and in a timely manner. e.Subcontractor will coordinate shop drawings with other trades as required. 008 Long Lead Material a.It is the intent of this Subcontract Agreement to furnish , for the work outlined by this Agreement , a “complete and finished product” for the Owner to the full extent required by the Contract Documents. Therefore, any components, labor and/or equipment necessary to complete the work of this Subcontract Agreement are deemed included as a part of the work of this Subcontractor and included in this price unless specifically excluded by the Contract Documents or this Scope. All work performed will be in strict accordance and compliance with the Contract Documents. Subcontractor agrees to submit an RFI for any questions, conflicts, etc. associated with their scope of work in a timely manner. 009 Interrelation of Documents a.It is the intent of this Subcontract Agreement to furnish , for the work outlined by this Agreement , a “complete and finished product” for the Owner to the full extent required by the Contract Documents. Subcontractor will give all notices and comply with all laws, ordinances, rules, regulations and orders of any public authority with jurisdiction over the Performance of the Work. Subcontractor will pay, promptly all fees, deposits, charges, damages, penalties or interest that may be claimed against or paid by Owner or Contractor due to any failure to comply with any such laws, statutes, ordinances, codes or regulations. 011 As-Built Drawings a.It is the Subcontractor's responsibility to maintain a current set of as-built drawings to be updated on a weekly basis. b.It is the Subcontractor's responsibility to provide a PDF copy of as-built drawings at the conclusion of their scope of work. 012 Closeout and Operating/Maintenance Documents a.Subcontractor will comply with all requirements of the Contract Documents as to Contract close-out and the provision of the operating and maintenance data. b.Subcontractor agrees to provide all required final documentation as specified. c.Supply attic stock with a transmittal and photos to be turned over to owner at completion of project as specified. 013 Warranty a.Subcontractor warrants that all materials and equipment furnished under this Contract will be new unless otherwise specified, and that all work will be of good quality, free from faults and defects and in conformance with the Contract Documents. All Work not conforming to these requirements , including substitutions not properly approved and authorized, may be considered defective. If required, Subcontractor will furnish satisfactory evidence as to the kind and quality of materials and equipment. b.All warranties as specified are to be submitted in a timely manner. 014 Clean Up a.Subcontractor will clean-up (on a daily basis) and deposit in dumpsters, at their expense, all waste and debris resulting from their operation, and remove from jobsite all surplus materials and equipment in connection with the as directed. In the event Subcontractor fails to complete the requested clean up on the first normal work day following written notification, Subcontractor agrees to accept all costs incurred, in having this work performed for the Subcontractor. b.Subcontractor will clean up and remove from the site all hazardous materials resultant from the performance of his Work . c.No hazardous materials will be placed in Contractor furnished dumpsters. The Subcontractor is responsible for this disposal. d.In addition to the required daily cleaning, this Subcontractor will provide one employee for (8) hours to participate in the weekly composite clean-up crew to be scheduled by the Superintendent. e.Subcontractor is responsible for cleaning of dirt and debris on roads and parking lots caused by their vehicles and deliveries. f.Subcontractor will provide for equipment and labor to clean these areas during their scope of work. Failure to do so will warrant the issuance of a Failure to Comply Notice. Further failure will result in the Subcontractor being supplemented. All direct and indirect costs incurred in supplementing this cleaning will be deducted from the Subcontractor's contract. g.Subcontractor will provide a final cleaning of finished surfaces upon the completion of their scope or work. h.Subcontractor will separate and dispose of trash in accordance to LEED requirements as specified. 015 Site Security a.Each Subcontractor will be responsible for the security of their own stored material, trailers, work, equipment, tools, etc. b.If any portion of temporary fencing needs to be removed by the Subcontractor while performing their scheduled work in a specific area, it will solely be that Subcontractor’s responsibility to maintain and replace any/all of the fencing to original condition, immediately after finishing their work or at the end of the work day whichever is necessary and appropriate. 016 Project Signs a.Project signs or advertisements of any nature will not be installed on the jobsite or structure without written permission. 017 Smoking, Eating, Radios a.Eating is prohibited on the construction site, except in the designated break/lunch areas outside the building, as designated by the General Contractor. Under no circumstances is eating in the building allowed. No drink cans or bottles are allowed in the building. Trash containers will be provided near designated break areas and Subcontractors are to deposit all trash in the containers immediately after their break/lunch. No glass containers or bottles are allowed onsite. b.Radios, tape players, etc. are not allowed on the premises. c.No tobacco products or electronic cigarettes will be allowed on the premises. 018 Drug Free Jobsite a.Subcontractor will maintain a drug free work place. b.No attire or paraphernalia promoting drugs, alcohol, or other inappropriate themes will be permitted. 019 Ground Fault Protection a.Subcontractor agrees to and will comply with OSHA Reg . Subpart K—Electrical 29 CFR 1926.404(b)(1)(i)(ii) Ground Fault Circuit Interrupters. Subcontractor will utilize Ground Fault Circuit Interrupters for his employees. Subcontract Agreement Date: Vendor No.: NORT28 Cost Code: 33-1000-00 S Project # 20029-0 - North Lake College Construction Sciences Building Contract Number: 001 b.Subcontractors are responsible to provide ground fault protection for their scope of work. 020 Site Conditions a.Subcontractor acknowledges that they have visited the site and are familiar with the existing site conditions. b.Subcontractor is advised that temporary use of the site for office trailers , storage trailers, site storage, and the like is at the sole discretion of the General Contractor. All costs of Subcontractor site office trailers , storage trailers, etc., including the installation and consumption costs of temporary electrical, water and/or telephone for them, will be the responsibility of each individual Subcontractor. c.The General Contractor will provide building corners and benchmarks only. The Subcontractor is responsible for layout for their scope of work. d.Provide for line and grade quality assurance surveying checks and layout for this scope of work, including all horizontal and vertical control. Maintain survey markers throughout the duration of scope of work if applicable. e.Subcontractor is responsible for locating and verifying all existing conditions. The Subcontractor will immediately inform the General Contractor of existing conditions not included in the Contract Documents. 021 Delivery and Storage of Material a.The Subcontractor will coordinate and schedule prior to delivery all material deliveries with the General Contractor’s Superintendent in order to maintain site access and minimize interference in work areas. Storage space will be limited, and the Subcontractor will be responsible for the proper protection of all materials stored onsite. The Subcontractor will relocate his materials at the direction of the Contractor if it interferes with the work of other subcontractors at no cost. b.Furnish temporary storage container(s) as needed to accommodate materials, tools, etc. for this scope of work. c.No compensation will be paid for storing or relocating material or equipment not coordinated with the General Contractor. 022 Parking a.Subcontractors are advised that parking of privately owned and company vehicles on the site will be permitted, but is limited and space is not guaranteed. Onsite parking for vehicles will be coordinated Contractor. b.All personal vehicles must be in be in accordance with current state and local regulations. c.If site conditions do not permit individual parking of all workers, then the Subcontractor is responsible for coordination of offsite parking or carpooling of personnel . d.General Contractor reserves the right to request and enforce the relocation of vehicles at it's discretion. 023 Insurance a.Should OCP become a requirement , a quote for the OCP must be submitted to Joeris for review prior to binding coverage . 024 Wage Scale a.Subcontractor agrees to wage Scale compliance and weekly certified payroll reports as required. 025 General Inclusions a.Subcontractor will perform scope of work and provide documentation in accordance to LEED requirements as specified. b.Subcontractor will attend all meeting and inspections as required per the Contract Documents. c.Subcontractor will protect all finished surfaces and make repairs to any damage caused by this scope of work. d.Subcontractor will provide for dust control measures as needed for this scope of work. e.Subcontractor will pay for any reinspections or failed work required due to failed initial inspections. f.Subcontractor is responsible for permits or fees as required for their scope of work. g.Subcontractor will provide access panels for their scope of work. h.Subcontractor will furnish and install all caulking and appurtenances associated with their scope of work. i.All work is to be coordinated with the General Contractor and predecessor/successor trades. j.Subcontractor will field measure and confirm all dimensions for their scope of work. k.Subcontractor is responsible for the repair of any fireproofing damaged by the installation of their scope of work. l.Subcontractor will furnish and install firestopping at their penetrations as required by the Contract Documents. m.Subcontractor will provide for traffic control measures as needed during this scope of work . n.Subcontractor will provide for general material handling associated with this scope of work, including scaffolding and stairs . o.Key Subcontractor personnel will not be removed from the project without consent/discussion with the General Contractor. p.Subcontractor is responsible for coordinating, laying out, furnishing, and installing sleeves/inserts for their scope of work. q.Subcontractor is responsible for the removal of any spoils created by their scope of work in a timely manner. Failure to do so will warrant the issuance of a Failure to Comply Notice. Further failure will result in the Subcontractor being supplemented. r.All direct and indirect costs incurred in supplementing this work will be deducted from the Subcontractor's contract. l Include all labor, material, equipment and supervision to complete the scope of work in strict accordance with the plans, specifications, addenda, and all ancillary contract documents, including, but not limited to the following: 026 Furnish all labor, material, and equipment to install the water utilities, including but not limited to, connection to existing water, piping, valves, meters, backflow preventors, copper tubing, meter vaults, fire hydrants, stainless steel risers, remote FDC, and DI fittings. Subcontract Agreement Date: Vendor No.: NORT28 Cost Code: 33-1000-00 S Project # 20029-0 - North Lake College Construction Sciences Building Contract Number: 001 027 Furnish all labor, material, and equipment to install the sanitary sewer utilities, including but not limited to, piping, manholes, cleanouts, and PVC fittings. 028 Furnish all labor, material, and equipment to install the storm drainage utilities, including but not limited to, connection to existing sewer, piping, grate inlets, manholes, junction boxes, fittings, and headwalls. 029 Subcontractor will provide a trench safety plan, fire underground design/permit, staking and layout, maintenance bond, and testing. 030 Subcontractor will sawcut and remove existing paving, replace paving, furnish and install flowable fill, and demo the exitsting headwall per the Contract Documents. Subcontractor will haul off all demoed material . 031 Subcontractor will camera each line at the completion of their scope of work. 032 Some water mains and valves are existing. Field verify location and depth of existing water utilities. Valve boxes shall be adjusted to the new finish grades. 033 Pay for and obtain all permits for work in public right-of-way and connecting to public facilities. 034 Install pipe true to alignments and grades shown on the Drawings, free of sags and unspecified bends. 035 Pipe shall be installed with pipe letters facing up so that it can be read from grade. 036 Install blue plastic warning tape that reads "Caution Water Main" in black letters every 4 feet and shall be placed approximately 12” above the top of pipe prior to the placement of subsequent backfill layers. 037 Install 4” wide aluminum tape at 12” below grade with all water distribution piping. 038 Install underground valves with stem pointing up and with cast-iron box. Provide valve stem extensions so that operating nut is set within 18 inches of finish grade. 039 Place a concrete thrust block on fire hydrant shoe opposite pipe connection, set against vertical face of trench. 040 Place a minimum of 5 cubic feet of gravel or crushed stone around base of fire hydrant to ensure drainage. 041 Fire service underground testing must be completed prior to connection with the fire protection system. 042 Provide a surveyor to stake locations of manholes, cleanouts and sanitary sewer main at a minimum 50-foot intervals. 043 Lay piping at lowest point of trench with spigot end pointing in direction of flow. 044 When work is not in progress, close open ends of pipe and fittings to prevent water, earth or other substances from entering the pipe and fittings. 045 Provide bedding for all sizes and depths of sanitary sewer pipe per the Contract Documents. 046 Manhole base shall be cast-on-site. 047 Green plastic warning tape that reads "Caution-Sanitary Sewer" in black letters every 4 feet shall be placed approximately 12” above the top of pipe prior to the placement of subsequent backfill layers. 048 Install detector tape with all sanitary sewer piping. 049 Provide connections for Landscape Drainage System. 050 Existing sanitary sewer and water utility lines are to remain in service at all times. 051 Uncover and verify the actual depth and condition of existing utilities prior to beginning the actual work. 052 All manholes, fire hydrants, valve boxes, etc. located in the grass shall have a 24"x24"x8" mow strip around it (Note 10, C5.00) 053 Provide 2' minimum vertical separation between water and sanitary sewer lines. 054 Implementation of the City of Coppell standard construction details per the Contract Documents. 055 Mechanically bore, hand dig, or air spade where new utility lines are constructed within the drip line of or within 15' existing trees that are to remain. Failure to do so will warrant the issuance of a Failure to Comply Notice. Further failure will result in the e.Subcontractor being supplemented. All direct and indirect costs incurred in supplementing a scope of work will be deducted from the Subcontractor's contract. f.Subcontractor will mobilize in accordance the project Schedule. g.Overtime required to meet schedule commitments will be the responsibilty of the Subcontractor. 005 Weekly Progress Status Meeting a.Weekly Progress Status Meetings will be held at the jobsite and conducted by the Contractor . b.Subcontractor’s Superintendent/Foreman and Project Manager attendance is mandatory. c.Weekly schedule updates including pull planning sessions will occur during the Weekly Progress Status Meeting . 006 Contract Documents a.Subcontractor is responsible for their scope of work as outlined in the Contract Documents. b.Contract Documents include, but are not limited to, the plans, specifications, addenda, and changes issued by the Architect . 007 Samples, Submittals, and Shop Drawings a.Subcontractor will furnish, at its cost, all samples, mock ups, and shop drawings for submission to the Architect in a timely manner to allow for approval and to prevent any delay to the Schedule. Subcontractor will remove mock up when requested. b.All submittals must be in accordance to the Project Documents and manufacturer's installation instructions. All Work not conforming to these requirements , including substitutions not properly approved and authorized, may be considered defective. If required, Subcontractor will furnish satisfactory evidence as to the kind and quality of materials and equipment. b.All warranties as specified are to be submitted in a timely manner. 014 Clean Up a.Subcontractor will clean-up (on a daily basis) and deposit in dumpsters, at their expense, all waste and debris resulting from their operation, and remove from jobsite all surplus materials and equipment in connection with the as directed. In the event Subcontractor fails to complete the requested clean up on the first normal work day following written notification, Subcontractor agrees to accept all costs incurred, in having this work performed for the Subcontractor. b.Subcontractor will clean up and remove from the site all hazardous materials resultant from the performance of his Work . c.No hazardous materials will be placed in Contractor furnished dumpsters. The Subcontractor is responsible for this disposal. d.In addition to the required daily cleaning, this Subcontractor will provide one employee for (8) hours to participate in the weekly composite clean-up crew to be scheduled by the Superintendent. e.Subcontractor is responsible for cleaning of dirt and debris on roads and parking lots caused by their vehicles and deliveries. f.Subcontractor will provide for equipment and labor to clean these areas during their scope of work. Failure to do so will warrant the issuance of a Failure to Comply Notice. Further failure will result in the Subcontractor being supplemented. All direct and indirect costs incurred in supplementing this cleaning will be deducted from the Subcontractor's contract. g.Subcontractor will provide a final cleaning of finished surfaces upon the completion of their scope or work. h.Subcontractor will separate and dispose of trash in accordance to LEED requirements as specified. 015 Site Security a.Each Subcontractor will be responsible for the security of their own stored material, trailers, work, equipment, tools, etc. b.If any portion of temporary fencing needs to be removed by the Subcontractor while performing their scheduled work in a specific area, it will solely be that Subcontractor’s responsibility to maintain and replace any/all of the fencing to original condition, immediately after finishing their work or at the end of the work day whichever is necessary and appropriate. 016 Project Signs a.Project signs or advertisements of any nature will not be installed on the jobsite or structure without written permission. 017 Smoking, Eating, Radios a.Eating is prohibited on the construction site, except in the designated break/lunch areas outside the building, as designated by the General Contractor. Under no circumstances is eating in the building allowed. No drink cans or bottles are allowed in the building. Trash containers will be provided near designated break areas and Subcontractors are to deposit all trash in the containers immediately after their break/lunch. No glass containers or bottles are allowed onsite. b.Radios, tape players, etc. are not allowed on the premises. c.No tobacco products or electronic cigarettes will be allowed on the premises. 018 Drug Free Jobsite a.Subcontractor will maintain a drug free work place. b.No attire or paraphernalia promoting drugs, alcohol, or other inappropriate themes will be permitted. 019 Ground Fault Protection a.Subcontractor agrees to and will comply with OSHA Reg . Subpart K—Electrical 29 CFR 1926.404(b)(1)(i)(ii) Ground Fault Circuit Interrupters. The Subcontractor is responsible for layout for their scope of work. d.Provide for line and grade quality assurance surveying checks and layout for this scope of work, including all horizontal and vertical control.
