Dear Mr. Lemp:
Enclosed for your review is a copy of the September 28, 2001 punch list generated from the
walk-thru of the referenced project. I have marked off all items that have been corrected to our
satisfaction. Item number 27 addresses "as-built" plans. I have those in my possession, and
although I have not finished my review, I am confident that any necessary changes will be
quickly corrected by NDM.
In updating this punch list, Public Services personnel brought forward three other issues. They
are as follows:
1. Remove copper riser east of 8" fire line vault.
2. Provide positive drainage away from the vault lid.
3. When the access road was built at the east end of Starleaf, a sanitary sewer manhole was
covered.
I hope this information is helpful. A copy is being forwarded to Ms. Jean Boney with Site
Concrete, Inc. Let me know if I can provide additional information.

Dear Mr. Lemp:
Based on a walk-through performed on Thursday, August 30, 2001 at 8:00 a.m. for the
referenced project, the following deficiencies remain:

5. At the inlet/jtmction box at the northeast comer, grout pipes and install plug on stub*out.
6. Plug stub-out from inlet on Line 'F'.
7. Clean, pour, invert and grout pipes in "Y" inlet at southeast comer.
10. Replace broken inlet lid.

13. I have received no compaction test results on the fill that was placed along the east slope in
the final stages of the project.
14. Verify the grades on the east slope,

16. Expose the 30" force main so that the support and suspected lealdng can be checked.

19. I have been advised that the inlet installed on MacArthur north of Colonial Dr. is pre-cast.
Pre-cast inlets are not approved for installation in public R-O-W.
20. Provide written conF~rantion from Carrollton-Farmers Branch lSD of their satisfaction with
thc condition of their property along thc south side of your development.
21. Install proper signage and slxeet markings on the left turn lane and crossover. If you have
any questions call City of Coppell Traffic Sign & Signal Dept. at 972-304-35~0.
22. Reinstall the existing irrigation system in the median in MacArthur Blvd. Contact Ron
McCain with Parks Dept. at 972-304-7001.
23. Install the permanent erosion protection and provide the associated bond. See enclosure

25. Provide the necessary maintenance bonds.
26. Provide certification for the back flow preventers.
27. Provide the required sets of was-built' plans. Call Eddie Johnson at 972-304-7018 for

Some of these items may have been corrected. I did want to compile all the concerns into one
