The owner has evaluated your bid dated January 26, 1996 for the above-referenced
project. You are the apparent successful bidder and have been awarded the contract. The
Contract Pdce of your Contract is Two Hundred Sixty-Nine Thousand, Nine Hundred Sixty-Six
and 00/100 Dollars ($269,966.00).
Six (6) copies of the contract documents accompany this notice of award. You must
execute these documents in accordance with the following instructions within 10 days of the
date of this notice of award, that is by Apdl 4, 1996.

Sign proposal in each copy.
Execute, sign and date Affidavit Against Prohibited Acts.
Execute, sign and date Bid Affidavit.
Execute, sign and date Affidavit of Safety Record.
Complete Performance Bond in each copy.
Complete Payment Bond in each copy.
Furnish Power of Attorney from Bonding Company in each copy.
Sign and complete Contractor Information last page of Agreement Between
Owner and Contractor in each copy.
Attach Certificate of Insurance in each copy.
If documents are executed by attorney-in-fact for contractor, furnish at least one
(1) copy of contractors Power of Attorney designation.
If contractor is a partnership, both partners should sign Proposal, Bonds and
Agreement.
If contractor is a corporation, provide Corporate Seal on Proposal and
Agreement. (Also attach evidence of authority to sign.)
Do not enter the effective date of Bonds or Agreement.
Do not staple or bind contracts.
Return all six copies to the Engineers Office to my attention.

A Maintenance Bond will be required by the City of Coppell and final payment shall be
contingent upon issuance of the Maintenance Bond to the City
