On this date, a representative of HBC Engineering, Inc. observed the injection of water at this
site. During the injection process, the following observations were recorded.

Surfactant added to each load. X
Injection pressure 50 to 200 psi at manifold. X
Injection downward in 18 inch intervals. X
Total depth of injection was 10 feet. X
Spacing of injection was 5 feet maximum. X
Iniection to refusal at each location. X
Comments: Jowell was notified that the surfactant level in the tank was not changing. Jowel insisted the
surfactant was going in. Later in the day, approximately 3:00 p.m., Jowell discovered that the tank
lines were clogged not allowing the surfactant to flow.
