The failure caused flooding in Denton Tap Road, a major six-lane boulevard through Coppell. Because of the flooding, I was called into the office to contact the contractor to get the situation resolved. I originally contacted Mr. Lee South because he was the person representing the contractor who applied for the Construction Development Permit in December 2001. I left a detailed message on Mr. South's cell phone instructing him to return my call immediately. When I did not receive a call back, I glanced through the file and noticed your name as being the contact for MYCON General Contractors. I then contacted you and requested that you provide people to resolve the issue. Because of the flooding in the street, it was difficult to determine what the actual cause of the failure was. During the course of our conversation, you informed me that it being late on a Sunday afternoon, you were unavailable to respond and that you were unsure what contact numbers you might have with you at the time. I informed you that because of the seriousness of the situation, I would instruct my Street Department to resolve the issue, but that all costs associated with the resolution would be billed back to you. I have now received the itemization of that cost. This cost will need to be paid before final acceptance of the project by the City of Coppell. Labor Cost $414.00 Equipment Cost 50.00 Total $464.00 The labor cost was comprised of two maintenance workers, Street Operations Manager, Assistant Director of Public Works and myself. The equipment cost was for pump use and truck use. If you have any questions, please feel free to contact this office. Sincerely,/~ Kenneth M. Griffin, P.E. Director of Engineering & Public Works
