Drive Coppell, TX 4M-0201003 Keith Houston CONCRETE PLACEMENT OBSERVATIONS As requested, a Giles Engineering Associates, Inc. (Giles) representative was present at the above referenced site to observe the placement of concrete, and perform field control tests. Observations indicated 20 cubic yards of Mix No. 494 AD mix concrete was placed this date for the sidewalks. The concrete was monitored and tested in regards to type, slump, temperature and batch to placement time. 1 set of 4 concrete cylinders were cast for laboratory strength testing. Prior to the placement of concrete, the placement area was checked for the removal of water, loose soil and unsuitable materials. Based on our observations and field testing, the concrete placed this date appeared to be in accordance with the project specifications.
