CONCRETE PLACEMENT OBSERVATIONS
As requested, a Giles Engineering Associates, Inc. (Giles) representative was present at the above referenced
site to observe the placement of concrete, and perform field control tests.
Observations indicated 12 cubic yards of Mix No. 549 concrete was placed this date for the driveway paving.
The concrete was monitored and tested in regards to type, slump, temperature and batch to placement time. 3
sets of 4 concrete cylinders were cast for laboratory strength testing.
Prior to the placement of concrete, the placement area was checked for the removal of water, loose soil and
unsuitable materials.
Based on our observations and field testing, the concrete placed this date appeared to be in accordance with the
project specifications.
Locations and elevations of tests were assessed in accordance with the accuracy and proximity of provided
survey control. Unless otherwise noted, the test locations and elevations were assessed by pacing and hand
level methods.
