Tracer tape shall be installed- 010.002 Centex Boring 07/02/2002 07/02/2002 Force Main contractor has the option to install casing in designated bores and/or to bore additional sections not indicated to be bored. However, if additional sections are desired to be bored it required city review and approval. All bores are to be dry bore and no jetting of backfill allowed. Bore will be 42" diameter, largest diameter of pipe fitting is 39.25". C-Con noted that they would use their equipment to push the pipe thru the bore. 010.003 Centex Layout / Coropaction 97/02!2002 D7J02/200Z Force Main staking will be performed by Centex Field Engineers and staking started today. Centex noted they had found a 2" elevation bust in elevation control provided by K$A Partners, the original St. Joseph Village Surveyor due to an old benchmark in the area. KSA has confirmed the correct - benchmark to be the same as Centex's. Turner Collie sraden to follow up on elevation control used on their design for possible coordination needed on remainder of piping system. Testing lab will be H.B.C. who is doing testing on St. Joseph village Utilities also and have been available as needed from a few times a day to full time. Compaction shall be 95~_ C-Con noted they would be using a sheept:ooC for compaction. City noted requirement for testing to be e-12" lifts and a maximum of 3oD feet horizontal. Centex noted typically the tests would be at 12" vertical and average of about 150 horizontal. 010.004 Centex General Site Coordination 07/0212002 07/02/2002 Prior to starting Force Main Centex needs to have portable JUL-U3-2~~i~2 13 17 CENTER CONSTRUCTION - DAU 972 233 2911 P.A3%04 OI0.005 Centex OIO.OOfi C-Con 010.007 Turner Collie Braden toilets on site. Trench Safety plan needs to be submitted. (Copy informally provided at meeting) City noted that Trench Safety is a big deal, c-Con noted that they intend to slope the excavation as much as possible. Existing Utility locates iunderground and overhead) need to be called for prior to excavation. City shall be called in advance to inspect pipe at delivery to site. C-Con noted this is currently scheduled for Monday, July 8th, that excavation should start on July 5th and work would take about 3 weeks. City noted that if existing utilities are damaged Rick O'Dell with the city shall be contacted immediately after the utility company is notified. If any unforeseen conditions or changes come up in' the field, they must be approved by the city prior to installation, order of contact with th,e city shall be Rick O'Dell, Larry Davis and then Ken Griffin as a last resort. All inspections shall normally take place Mon-Fri, 8:00 to 5:00. Tf inseections are required outside this time, city must ha notified in advance. IE working on Saturday, it must be faxed into the city by noon on Wednesday each and every week. if cancelling a Saturday schedule, it must be cancelled by 3:OOpm on Friday or the city will come on Saturday anyway and expect payment for inspection. C-Con noted that they will start work each day at 7:OOam. Specific Site Coordination 07/02/'1002 07/]012002 Drior to installation of Force main pipe across City of zrving 54" water line, provide written notice to them, ao they can have a repre9antative theta when installing if they desire. C-Con noted that the SJV storm laterals that will cross the Force Main will be installed after the Force Main. AT&T Broadband indicated that their service is all on pales along Sandy Lake Road. City noted that C-Con is to have patch kit of the same liner material on site to repair all damage to lining as pipe is installed. Documents /Submittals 07/02/2002 07/08/2002 There were no questions about the Force Main documents. City noted that if their are any conflict within the documents City Standards shall take precedence. The city noted they have not received the following submittals in addition to the Trench Safety plan; Concrete Mix design for Structures, A.v. valves and Manhole Frames. C-Con to provide submittals immediately. Testing 07/02/2002 07/10/2002 Hydrostatic testing of Force Main piping is preferred and specified, however city noted that air testing may be allowed. Ted Sugg with Turner Collie Braden to verify with Manufacturer their recommendations on testing. Tt was noted that this contract was for only the portion of this system which crosses the St. Joseph Village project site and that the full length of this piping system would not be in service for about another year. C-Con noted that they would test the entire length of this contract (approx. 2200 feet) at completion. Oi0.008 Centcx Pay Application 07/02/2002 07/02/2002 Centex will submit the pay application each month. Current procaduxe on St. Joseph is to fax pay app to Architect and Owner prior to last Tuesday of month for review, then meet at site to review installation at noon on Iast Tuesday of month. Centex will forward pay app to Turner Coliie and Braden at same time for their review and provide notification of site walk. 010.009 Centex Completion Documents 07!0212002 07/OZl2002 The city noted upon completion of the Force Main a final walkthru would be required to review As-guilt drawings. The 2 year Maintenance Sond will begin at Final Acceptance. The above constitutes the writer's understanding of the items discussed and the decisions made.
