Comments: The manhole located at station 4 +18 on DeForest Court is approximately 4 to 5 inches
higher than the driveway approach that was poured two feet away from the manhole making It
impossible to transition the sidewalk around the manhole. We have been asked by the developer to
lower the manhole and rather than remove the cone and a 2' riser section and install an 18" riser
section, we are proposing to use. a top flange ring as shown in the Bass Hayes catalog, page 2,
#380 -24. This would allow us to achieve the desired top elevation without extensive excavation around
the new gas and electrical lines that have been installed.

verify horizontal and vertical location of all uttlitles prior to start of construction.
Notify owner of any potential conflicts which may exist between the existing
utilities and construction plans.
4. Any damages that may occur to real property or existing improvements
shall be restored by the Contractor to at least the some condition that the
real property or existing improvements were in prior to the damages. This
restoration shall be subject to the Owner's approval; moreover, this restoration
Shull not be a baste for additional compensation to the Contractor. Restorotion
shall include, but not be limited to, regressing, revegetation, replacing fences,
replacing trees, etc.
5. The Contractor shell abide by all opplicable federal. state. and local laws
governing excavation. The contractor shall provide detailed plans and
specifications for trench safety systems thot comply with applicable lows
governing excavation.
