Mr. Valk:
Rick O'Dell has brought it to my attention that you do not intend on conducting the tests on the fire
lane as was agreed on in the meeting that we had last Friday, July 25, 2003, at 11:00 a.m.
The tests that were requested by the city include:
• A full soil analysis of the `flex base' subgrade. It is suspected that the subgrade does not qualify as
true flex base. Further, densities were not taken on the subgrade prior to placing the concrete.
• Core samples of the concrete paving. Concrete cylinders were not properly prepared, documented,
and tested as is required by the city's construction details, specifications, and subdivision
ordinance. In addition, it is suspected that the concrete paving does not meet the minimum
thickness and/or strength for the `flex base' subgrade.
• Density testing of the `flex base' subgrade. Prior to pouring the concrete for the fire lane, density
tests were not properly conducted and documented on the subgrade.
In addition to the above deficiencies in testing and documentation for the constniction of the fire lane,
the concrete mix design indicates that fly ash was included. The use of fly ash is specifically prohibited
in concrete pavements within the city (see attached).
The Building Inspections and Fire Departments have been notified of your refusal to comply with the
city's requirements. All necessary department approvals and the Certificate of Occupancy will not be

Concrete Note: Pavement thickness and strengths are minimum requirements based on 28-day
test, and flexural tests shall be center point loading. Traffic projections may
require additional thickness of paving on major thoroughfares.
In addition to the above, all hand-pours and irregular sections shall contain a
3600 PSI concrete with a minimum of 6 sacks of cement per cubic yard. All
concrete mites shall contain crushed rock aggregate. Fly ash will not be
_ permitted for concrete pavement mixes.
