t1 looking through our FEMA files it appears that this property is shown to be within the 100 year
floodplain. It is very possible that an elevation certificate indicating that the structure was constructed
at ]east 2 feet about the base flood elevation exists in our Building Inspections Department. You can
verify this information with them at 972-304-3500.
Several property owners within the Creekview Subdivision have elected to go through the FEMA
LOMA (Letter of Map Amendment) process to remove at least the structure portion of their property
from the floodplain to prevent the requirement for flood insurance in the future. I would recommend
that your client visit the FEMA website at www.femagov. The proper form can be downloaded from
the website and submitted to FEMA for consideraton. The form is called the MT-EZ.

My name is Beth Clay and I am with Commundy Credit Union. We have a customer whose property
shows it is in a Flood zone area and he is disputing this. I talked ~m~th Teresa on Monday regarding this
issue- From your City maps if you could verify if this house is in a flood zone area- Attached is his
survey. it is let 21-BLK 8--at 2fi5 Penuel Dr. He is asking if we could yet a reply today, so if that is
