On this date, HBC/Terracon's technician arrived at the above referenced jobsite as scheduled for
in-place density tests. After arriving at the jobsite, HBC/Terracon's technician was notified that
in-place density tests were cancelled due to the contractor was not ready for testing.

**REVISED
Services: Drilled pier observation.
On this date, HBC/Terracon's technician arrived at the above referenced jobsite as scheduled for
drilled pier observation. See the attached sheet(s) for recorded data.
NOTE:
**Pier 19 was concreted half way, then the casing was removed because there was
no concrete to place into the pier hole and the next concrete truck would arrive at
the jobsite for two hours. HBC/Terracon's technician informed the superintendent
that all the water and mud needed to be pumped out of the pier hole before the
the rest of the pier hole could be concreted. The contractors did not pump the
water and mud out of the pier hole. The concrete was placed with the mud and
water still in the pier hole. The first truck of concrete filled the pier hole to within
approximately 9 feet from the ground level. The superintendent was notified that
the contractors had not pumped the water out of the pier hole before the second
truck of concrete was placed.
**This repod has been REVISED on 08~29~03 by changing the Time Concrete
was Placed for Piers 4 and 19 (see attached sheet).
