We request a change order on the pro~ect (DR 93-06), to cut
and DiOCK exlsting 8" water i!ne, on Doth s/des o2
cuivert,!n the north quadrant of Betnei School Road.
Remove water ilne to allow piacing of new culvert to the
west s!de of old box culvert. Then piac~ng new 8" D.I. wa~er
ilne over the top o~ Oot~ culverts and tie!ng DaCE lnto
existing water line. Tnls change order also lnciudes the
moQificat~on to t~e DOX Culvert at t~e sewer manhole on the
south quadrant, west s~de o2 0ox, and using asphalt to open
the sout~ naif o2 Betnei School Road to t~a~fic, wh~ie
rema!nder o2 culvert, remove and replace sldewsik north side

We propose to do the complete change order for t~e lump sum
o2 n!neteen tnousan~ seven hundred seventy five dollars and
no cents(S!9,??5.00) .
We are request!ng an a~!t!onai twenty(20) ~ays to ma~e
correct!ohs requlre~ to make the c~anges.
