RE: Work Order # 5254618 (CiCi's Pizza - W. Bethel Rd.)
Dear Ms. Cantu:
On Friday, March 12, 2004 I observed your contractor for the above referenced project to be in violation
of several City of Coppell permit requirements. Hereafter listed are the specific violations noted:
1. Erosion and Sedimentation Control Code - our ordinance was designed to minimize erosion and
sedimentation from private property onto public rights-of-way, making it unlawful to permit a public
erosion nuisance and providing for a penalty of fine not to exceed the sum of $500 per day during
which a violation occurs. This ordinance was not being followed as an unsecured dirt pile was created
near the road. The permit application clearly states that erosion control fences are to be installed prior
to beginning work.
2. Working time requirements - working time allowed in any roadway is limited to the times stated on
the permit form. Do not work outside these parameters.
3. TMUTCD barricading requirements - contractor did not following proper roadway closure and/or
signage guidelines. (see comments on diagram)
4. Possible peripheral damage to adjacent property - the ejection of drilling mud from the bore hole
has mud flowing to the surface at the joints of brick pavers in the easternmost driveway on Bethel
Road.
5. Blockage of fare lane - during my inspection, I observed the driveway to the west of the above-
mentioned driveway being blocked by material and equipment. The Fire Marshal was notified and the
lane was eventually reopened after an unacceptable length of time.
We respectfully request that you immediately contact your contractor, B & B Backhoe and make them
aware of the requirements that must be adhered to during any work activity of any kind taking place in the
City of Coppell. Please contact this office so that we can discuss their compliance to all applicable
requirements.
