Utility Construction Plan Review Comments
Dear Mr. Trudell:
Thc City of Coppcll has reviewed the referenced plans and has thc following comments to offer:
1. Provide a completed permit application for this installation. A permit form is attached for your usc.
2. I.abel or provide cover sheet with date on all future plan transmittals.
3. Pay the appropriate construction inspection fee in the amount of 4% of the cost of this work.
4. Provide a ban-icadc plan for approval.
5. No work can bc done in thc roadway Mom Fri. until 9:00 a.m. and must stop by 3:30 p.m.
6. lnslall traffic rated plates over thc asphalt roadway.
7. Keep Bethel Road open to traffic at all times and maintain the roadway flee of mud/dirl.
8. Submit a trench safety plan.
9. Usc flowablc fill in trenches tinder roadways.
I0. Provide detail on proposed street repair for approval.
11. Use City of Coppei1 Standard Construction Details.
12. Provide a 4" sanitary sewer cleanout at the property line.
13. Are there any existing services to be removed?
14. Provide and maintain an approved SWPPP on this project.
15. Dry bore to thc existing sanitary sewer main, then open cut.
16. Page two of this transmittal refers to the project location as 3000 Bethel Rd. Should it be 300
Bethel Road?
Please make thc necessary changes and submit two sets of revised plans to the City for approval, if
