PAVECON, INC. offers to furnish all labor, materials and equipment
required for the performance of the following described work in
connection with construction or improvements at above address.
Pavement improvements:
The work is to be performed as follows:
Repair failures in Bethel Road, there are four locations: A)Westbound @
Meatherstone approx. 75 foot by 11.5 foot B) From joint just east of
Coppell Deli approx. 600 feet by 11.5 foot to the west on the east
bound lanes C) West of Loch West bound approx. 200 foot by 11.5 foot
D) East bound @ Manhole at Freeport Parkway approx. 100 foot by 11.5
foot.
Prices are based on completing Excavation, fabric, backfill and CTB in
a 50 Foot by 11.5 Foot section per each working day :

Has the failures (approx. 975 if x 11.5) to a depth of
3 feet (approx. 1,246 sy). Additional grade variation to be
determined by Engineer and billed by unit price.

Owner shall be responsible to locate all buried utilities li~es
structures, a~d to cleerly identify a~d ~ark all lines and sub
stx~ct~res, by calli=g 1-800-DIG-TESS prior to co.~p~ce~nt of the w~rk
by PC~. By accegting Chis proposal, Owner agrees to indemnify and to
hold PCH harmless from all claims, including its reasonable attorneys'
fee incurred in defending any claim, resulting from damages to any
utility line or sub structure not identified or clearly marked. If
owner shall clearly mark all utility lines or sub structures and such
lines or sub structures are damaged by PAVECON, INC. PAVECON shall
assume responsibility for all repair costs associated with any damage
to such buried items.
Cost of ~ity permits (if applicable) are not included in total cost and
will be paid by owner.
if the foregoing meets with your acceptance, kindly sign and return the
attached copy of our proposal. Upon its acceptance it is understood
that the foregoing, including the terms and conditions set forth and
attached hereto and incorporated herein for all purposes, will
constitute the full and complete agreement between us.
