Dear Dave:
Please provide us with a proposal for the above referenced
subject. We have enclosed a plan (Exhibit A) and a section
(Exhibit B) for your use.
The items required for the change order shall be furnished and
installed in accordance with the contract documents and the
attached exhibits. Total lump sum cost for this item shall
include all work and materials necessary for furnishing and
installing the water mains and appurtenances shown on the plan,
including all excavation, dewatering, bracing and shoring, trench
safety, bedding material, pipe laying, jointing, backfilling,
compaction, thrust blocking, testing, disinfection and any other
work that is required to complete the installation.
Please complete the attached line item summary sheet, in
duplicate, and return to us. Should you have any questions, or
need additional information to prepare your proposal, please
contact us.
