JACOBS, P.E Jtme 29, 2000 Transmitted via Facsimile Advantage ontract Services, Inc. 7150 eda Genoa Rd. ~77075 Re: City of Coppell Repainting of the 1.5 Million Gallon Elevated Tank Gentlemen: A final walkthrough of the Repainting of the 1.5 Million Gallon Elevated Tank was conducted on Thursday, June 29, 2000. The following representatives were in attendance: Glenn Hollowell, P.E. Tedd Dutt: Gary Hendricks, P.E.: David Cadrill: City of Coppell City of Coppell Shimek, Jacobs & Finldea, L.L.P. Advantage Contract Services, Inc. I & C Sales The following items were found to be either incomplete or in need of correction: 1. The power conduit, wires and connections for the motor operated valve are not installed. We were unable to verify the proper operation of the valve nor its operation through the SCADA System. 2. Once the valve is powered, the specifications require a written test report on its start-up, training and operating condition. 3. The Operation and Maintenance manual for the motor operated valve has not been submitted. 4. The grating around and over the new motor valve requires modification to fit the new valve configuration 5. The concrete walls for the pipe and valve gallery in the tank have been removed for a distance of roughly 2 feet on both sides of the gallery. In addition, a 2'x2' portion of the tank' s concrete floor has been removed on both sides of the new valve. The gallery walls and tank floor must be returned to original or better condition prior to final acceptance of this project. 6. Revised drawings on the SCADA modification have not been submitted and are required. 7. The portable latrine needs to be removed from the project site. 8. The contract documents require the contractor return the site ground cover using solid block sod. 9. The removed gate valve is to remain the property of the City and shall remain at the tank site. Final Walkthrough June 29, 2000 Page Number 2 As a reminder, the project specifications require a 12 month and 24 month inspection of the tank along with any necessary repairs. Once the punch list items are complete to the City's satisfaction, we will prepare the final pay request and forward the Affidavit of Final Payment and Waiver of Release for your signature.
