NOTICE OF INSPECTION DUE
The Texas Architectural Barriers Act (Article 9102, T.C.S.) requires inspection of subject facilities to ensure compliance with
the Act. The referenced project may be inspected either by Texas Department of Licensing and ReguJation (TDLR) staff
or a Registered Accessibility Specialist (RAS) endorsed to perform inspections.
If you prefer to have this project inspected by TDLR, please complete the applicable information below and return this entire
notice with check\money order for the inspection fees, payable to TDLR. within 20 days receipt of this notice,
If you choose a RAS, prease contact the RAS first, Then complete the applicable information below and return this entire
notice to us with check\money order for the $100 inspection Filing Fee, payable to TDLR, within 20 days receipt of this notice.

If this project is not complete, has been cancelled, or has changed ownership, please complete applicable information below.
