The eastern driveway at Minyards needs to be adjusted by 1 1/2, by raising the front
section and lowering the back section.

At Heatherglen need to lower the front side of the crosswalk by approximately 3".
Send letter back to Accessology, Inc. stating that a 30 x 48 area provided for PED
button and that cross button is within reaching distance.
(that comment should be eliminated).
4. Southwest intersection of Sandy Lake/Heartz, while not on the list, it does need to
have a crosswalk created to the ramp with contrasting colors, particularly the
north/south direction.
Southwest intersection before Sandy Lake/Heartz need to raise the sidewalk headed
v/~est up to the level spot of the curb ramp. Rebuild the curb ramp possibly band it in
concrete with dark gray pavers to create contrast. This will create the level floor
space for the PED Buttons.
'/~0~9~, 3'q. way at Sandy Lake the grooved areas of the crosswalk need to be stained.
~/~Simmons west, the west intersection need to remove curb ramp, and rebuilt it.
Lowering it to create a flat landing area for the ramp and then stain accordingly. The
cross slope between the curb ramps needs to be removed and replaced.
X/Simmons west, the east side of west Simmons bring the color of the ramp straight up
(don't color the whole section of the ramp).
~Simmons east, need to remove the whole intersection back to where we previously
changed the grade to direct more water to Sandy Lake to create a flat area.
~/Dobecka need to paint the north half of both ramps (not entire ramp)
x/6. Castlebury southeast corner, nothing to be done other than painting the ramp in its
entirety. Paint ramp up to the southeast comer, paint ramp beyond the actual ramp
itself to the level spot about 2 1/2 feet beyond the center of the ramp.
'/Sandy Knoll and Sandy Lake need to paint the ramps.
X~ighland Meadow/Sandy Lake southwest corner, need to rebuilt the top of the ramp
to a differefflie color.

~Iighland Meadow/Sandy Lake southeast comer, need to create a level area,
potentially bring the sidewalk around, then build ramps off of sidewalk.
/Holly south/Sandy Lake need to paint the ramps straight line up both sides to align
with the sidewalk.
v/Nash/Sandy Lake southwest comer, need to paint, southeast comer need to remove
ramp create flatter sloping ramp and flat section of sidewalk and then paint.
'~9. Moore/Sandy Lake southwest comer, need to create the clear space by building the
curb and raising the sidewalk. Need to build a sidewalk all the way around connect
the ramps and then put in 2 curb ramps 3 foot wide matching up with the crosswalks.
Include the clear floor space as a repair don't include as a variance. 48" should be
running the north/south, which gives a longer reach distance.
10. Sandy Lake/Moore southeast comer, need to adjust the sidewalk and create a level
landing at the PED Button, 48" running east/west. Raise the crosswalk, crossing and
x~/ the ramps at the western most driveway into the shopping center on south side of
Moore Rd and Sandy Lake both ramps need to be removed and replaced cross slope
of the driveway needs to be flattened. Middle driveway at the shopping center needs
to be painted along with the eastern most driveway. Sidewalk on the east side of the
alley east of the shopping center at Moore/Sandy Lake needs to be painted / look at
grooving for texture differences.
~?& paint the crosshatched area of the crosswalk. Request variance for crossing, if not
approved, then need to remove and replace.
Sandy Lake/Samuel southwest comer, need to remove the existing ramp make it flat
with the street, create a ramp coming up on the west side of it to a landing ramp and a
ramp from that landing area to the sidewalk, then remove the concrete on the east side
of the ramp, create a flat landing area at the PED Button, and then create a steeper
piece of concrete to connect the two, that would be a pedestrian, but not ADA
accessible area.
(no variance on this one, needs to be fixed)
Sandy Lake/Samuel southeast comer, the clear floor space issue #17 goes away. If
we rebuild the ramp on the southeast comer probably need to put in a series of ramps
and tums to get the appropriate slopes.
,~andy Lake/Mockingbird southwest comer, need to paint the grooved area of the
ramp.

get the right slope on it.

Sandy Lake/Mapleleaf the crossing between the ramps need to remove and replace to
get a 3 foot flat area or 2% slope area. Potentially try to rework the pavers to get to a
3 foot area that is flat or is 2% cross slope. Color of the ramps 3~foot wide
leading into the sidewalks
Sugarberry/Sandy Lake need to color the ramps both sides.
MacArthur/Sandy Lake southwest comer, need to pour a concrete pad area by the
PED Buttons.
MacArthur/Sandy Lake southeast comer, need to pour a concrete pad area by the
PED button.
Clear space area, need to remove the concrete area around the pole at the northeast
comer and create a clear landing area.
Kroger, the eastern most driveway I am not sure how to approach, hopefully will get
a variance if not we need to take out all the walls, sidewalks, ramps, entrances,
~/driveways, about another 25-30 feet of the Kroger parking lot and lower everything
down to make it work.
Kroger, the east side of the main driveway need to paint a 3-foot area across the
~/crosswalk to the level portion of the sidewalk approximately 6-7 feet away. The
western most driveway at Kroger leave the existing ramps and pavers alone but need
to cut in two additional ramps closer to the street and the flat area and paint
accordingly. Potentially look at the eastern most driveway to cut in two additional
ramps.
v/~ountainview/Sandy Lake need to stain the ramps on both sides and paint all the way
to second joint outside the grooved area of both sides up to the flat area.
28. Need to paint the portion of the sidewalk that slopes too much as a curb ramp.
Note that in the letter to Accessology, Inc. on both sides of Meadowglen.
29. On the cross slope of the ramps either cross walk between the ramps either pull out
ramps, rebuild, potentially cut in two additional ramps closer to Sandy Lake Rd.
~Meadowmn/Sandy Lake need to paint the ramps beyond up to the sidewalks where
they are flat.
~teeOrgetown Plaza eastern most driveway request a separate variance for this due to
chnical issues associated with trying to connect to existing driveways, exiting
utilities, and cross slope of Sandy Lake Rd.
-/Georgetown Plaza middle driveway need to stain a 3-foot wide area across the ramp
not the entire ramp on sides of the middle driveway. On the ramps on the both sides

of the middle driveway sawcut out the grooves further into the flat landing area.
Remove eastern ramp at the western most driveway to Georgetown Plaza, put in a 3-
foot ramp connected to a sidewalk. The westernmost driveway to George Town
[~/'Plaza need to rebuild the sidewalk to come up to the brick pavers. Build an additional
ramp off of the east side of it and on the west side. Probably need to rework the brick
pavers to get a 2% cross slope.
32. Samuel/Sandy Lake northeast corer ????meets requirements

Sandy Lake/Moore northeast corner, need to build a 30 x 48 pad on the east side of
x~the traffic signal for the PED button, 48 running north/south. Need to provide
contrasting color at the top of the ramps
36. Sandy Lake/Moore northwest corner need to build a pad on the north side of the
traffic signal 48 running east/west, on the north side of Moore potentially both sides
x/ of the intersection maybe can pour 3-foot wide sidewalk behind the existing paver
bands to take care of the contrasting color issue.
Aandy Lake Baptist Church need to stain the ramp on the west side of the driveway to
the. Need to groove the sidewalk section to provide a texture difference.
~/tRejoice Lutheran Church driveway need to put on blanket variance request. If have
o fix need to lower the crossing and stain both sides of the driveway approach as a
ramp.
L//SAandy Lake/Holly north part of the blanket variance request. In letter to
ccessology, Inc. appears to be within construction tolerance and to remove from list.
Need to stain both sides of the ramp and provide some texture differential.
~. Sandy Lake/Lodge 4 ramps on the north side, request a variance. If not, have to take
out and rebuild the ramps to provide 4 ramps and a flat area with contrasting color.
x/nTrailwood ramp northeast side, need to stain the textured area of the ramp. On the
orthwest side of Trailwood/Sandy Lake don't color the textured area of the ramp but
color the first panel of the sidewalk on either side of the ramp.

probably on the east side of the traffic signal. Also need to provide color
differentiation at the top of the ramp.
Tom Thumb 2 driveway ramps: The eastern most section of the eastern driveway
~//need to stain the second sidewalk panel away from the cross hatched (do not need to
stain the crosshatched area). On the west side of the eastern most driveway need to
stain the south side of the cross hatched area or a 3-foot section through there. At the
western most driveway, need to stain the ramp and the first sidewalk panel on the east
side of the driveway and on the west side need to stain the ramp.
