The City of Coppell has received and reviewed the plans submitted for an installation at the
above referenced location and we have the following comments to offer:
1. The permit application does not include contractor inf'o~Tnation. The City will require the
contractor name, contact person, and 24-hr. emergency contact number prior to issuance of
the permit.
2. Contractor shall pay special attention to pages 3 /,2 4 of the permit application. The City of
Coppei1 will require conlbm~ance to all items listed on page 3. Page 4 is to be completed by
City Streets and Utilities divisions personnel and returned to this office prior to release of any
additional pemaits to MCI or the contractor mvolvcd.
3. Maintain a minimum 5' horizontal clearance from Cit? water, sewer or storm drain facilities.
4. Maintain a minimum 2' vellical clearance, preferably under our waterlines as opposed to
above.
5. City of Coppell requires full depth saw cuts of concrete and silicone sealant for all concrete
seams.
6. Lane closures may only take place between the hours erg:00 a.m. and 3:30 p.m.
7. Construction Drawings shall reference City of Coppell specifications and requirements.
References to other cities' specifications shall be removed from the set. See mark-ups
throughout the drawings for additional comment
This pemdt can be approved xvith these referenced ~equircments as condilions, however the
permit cannot be released until conh-actor inlbnnation is provided, Prior to beginning work
please contact the City inspector and the Utilities dixision; a pre-construction conference shall be
scheduled at our office. Included with this l;ax arc pages 3 and 4 of the permit as referenced in

UTILITY RELOCATION CHECKLIST FOR WORK WITHIN
CITY OF COPPELL RIGHTS-OF-WAY OR EASEMENTS
Listed below are minimum requirements for utility relocation projects:
1 A copy of the valid approved CityofCoppelI permit is required on thc project at ail times.
2. Nolle, adjacent property owners (commercial & residential) of project timeline; provide them with contact
names and phone numbers.
3. Proper erosion control measures must be taken.
4. Follow Texas MUTCD standards for work in traffic zones.
5. Sod is to be replaced when necessary.
[n'igation repairs must be performed in a timely manner.
7. Follow OSHA recommended trench safety at all times.
8. Repair existing utilities that are damaged, including but not limited to clean-outs, water valves, and SS
manholes.
9 Densities may bc required in trench lines (ASTM).
] 0 Care and repair of street signage is the responsibility of the contractor.
11. No work is to be done in collector roadways bcfore 9:00 a.m. or aftcr 3:00 p.m.
12 Prior notice of work activity on weekends and holidays must be in writing.

14. Contaminants must be removed from work area daily and disposed of properly outside the City.
