RE: Right-of-Way Use Permit Application in Coppell, Texas

Dear Mr. Michael:
I have reviewed the drawings submitted as the preliminary permit application packet for the
proposed Verizon FTTP system in the City of Coppell, and I have the following comments to
offer:
1. Please provide typical details and/or pictures of installations of the different hand h°les and
FDH cabinets.
2. Screen out seal and signature of the engineer responsible for the original utility work. That
engineer has no design responsibility for this work.
3. How will individual homes be fed from the pull boxes? Will any additional work in the
ROW be required?
4. What type of notification and/or signage are you proposing for the residents of the
subdivisions?
5. Ensure that appropriate BMP's for erosion and silt control are employed at all times.
6. Clean up shall take place immediately after completi°n °f the installati°n'
7. All disturbed areas will be re-established with block sod of the same variety of grasses, or
equivalent landscaping material of whatever was disturbed.
8. Any utilities disturbed, including private irrigation systems, will need to be repaired to the
owner's satisfaction in a timely manner.
9. No permits will be issued tmtil such time as we have a 24-hour emergency contact number
for the contractor performing the work.
10. No work shall begin until such time as a pre-construction conference has taken place with
representatives of the City, the contractor, and the owner (Verizon).
11. We will accept black and white print drawings as long as the work to be completed is
highlighted to draw attention to it.
12. We will require 5 sets of drawings for each permit application.
